Abstract

BAKALÁŘSKÉ PRÁCE Jméno: Eva Zemanová Vedoucí práce: Kateřina Svěcená Název bakalářské práce: Case management z pohledu ergoterapeuta u osob se spinálním poraněním, Case Management from the point of view of the occupational therapist in persons with the spinal cord injuries Abstrakt bakalářské práce: Bakalářská práce se týká tématu case managementu u spinálních pacientů. Zaměřuje se na pojem case management, tedy koordinované vedení případu. Informuje tedy o možnosti, jak propojit služby s uživatelem a zkoordinovat jednotlivé fáze rehabilitace u spinálních pacientů, jejichž léčba je zdlouhavá a rozčleněna do mnoha zařízení. Dále zjišťujeme, jaké služby case manager uživatelům nabízí. Momentálně je hojně využíván v zahraničí, v naší republice se zatím rozvíjí postupně se značným zpožděním, zjistíme tedy, zda by se uplatnil i v našich podmínkách a jakým způsobem. Dozvídáme se zde systém organizace rehabilitačních zařízení a jejich propojení, informace ke spinálnímu programu v české republice i v zahraničí. Nedílnou součástí je zapojení ergoterapeuta do tohoto procesu a zhodnocení informací o tom, jakým způsobem se ergoterapeut může do case managementu zapojit. It focuses on the concept of case management. This is a coordinated manage of case. Informs about the possibilities to connect services with the user and to coordinate the various phases of rehabilitation for spinal patients whose treatment is lengthy and takes place in many medical facilities. Furthermore, there also we find out what services the case managers offer to users. Case management is widely used abroad but in our country is developed gradually and with considerable delay. We will find out whether it be used in this patient group in our country and how.
