Cíl studie: Zhodnotit efektivitu vybraných dopplerovských parametrů jako testu predikce diskordantního růstu (DR) a porovnat ji s efektivitou rutinně používaných ultrazvukových biometrických parametrů. Typ studie: Prospektivní klinická studie. Soubor a metodika: Soubor tvořilo 63 dvojčetných gravidit s celkovým počtem 142 vyšetření. Byly studovány rozdíly mezi oběma plody v dopplerovských parametrech (arteria umbilicalis - AU, arteria cerebri media - ACM, cerebro-placentární poměr - CPR) a biometrických parametrech (biparietální průměr - BPD, obvod břicha - AC, délka femuru - FL, odhad fetální hmotnosti - EFW). Současně byla sledována i rezistence uterinních arterií. Za DR byl považován rozdíl porodních hmotností od 20% výše. Efektivita interfetálních rozdílů sledovaných parametrů byla zhodnocena pomocí ROC analýzy. Výsledky: Biometrie provedená do 3 týdnů před porodem byla pro predikci DR růstu přesnější než dopplerovská flowmetrie (AUC pro EFW 0.99; pro AU 0.56; pro CPR 0.71). Při vyšetření v rozmezí 3-6 týdnů před porodem byla efektivita biometrických a dopplerovských parametrů srovnatelná (AUC pro EFW 0.79; pro AU 0.81; pro CPR 0.81). S odstupem více jak 6 týdnů před porodem nebyla již biometrie efektivní, z dopplerovských parametrů predikoval DR jen CPR (AUC pro EFW 0.62; pro AU 0.56; pro CPR...
Objective: To evaluate the efficiency of selected doppler parameters as a test for the prediction of twin growth discordancy (GD) and to compare it with the efficiency of routinely used ultrasound biometrical parameters. Design: Prospective clinical study. Methods: Set was formed by 63 twin pregancies. Intertwin differences in Doppler parameters (umbilical artery - AU, middle cerebral artery - MCA, cerebroplacental ratio - CPR) and biometrical parameters (biparietal diameter - BPD, abdominal circumference - AC, femur length - FL, estimated fetal weight - EFW) were studied. Simultaneously, uterine arteries impedance was assessed. GD was identified by the birth weight difference from 20%. The efficiency of intertwin differences in studied parameters were evaluated using ROC analysis. Results: Biometrical measurements performed within three weeks antepartum were in prediction of GD more accurate than doppler parameters (AUC for EFW 0.99; for AU 0.56; for CPR 0.71). Three to six weeks antepartum, the efficiency of biometrical and doppler parameters was comparable (AUC for EFW 0.79; for AU 0.81; for CPR 0.81).
