Screening rizika a diagnostika podvýživy u pacientů s Huntingtonovou nemocí v klinické praxi nutričního terapeuta

Abstract

Úvod: Huntingtonova nemoc (HN) je dědičné neuropsychiatrické onemocnění, které má fatální průběh. Projevuje se zejména hybnými symptomy a psychickými změnami. U HN, podobně jako u jiných neurologických onemocnění, často dochází z mnoha příčin k rozvoji poruch výživy. Cíle: Prvním cílem našeho výzkumu bylo provést nutriční screening a vyšetřit stav výživy u souboru pacientů s HN a zjistit v jakých parametrech se liší od zdravé populace. Druhým cílem bylo zhodnotit senzitivitu a specifitu screeningových nástrojů Malnutrition Universal Screening Tool (MUST) a Global Leadership Initiative on Malnutrion (GLIM) v diagnostice podvýživy u pacientů s HN. Soubor a metodika: Soubor pacientů i kontrolní soubor tvořilo 30 osob, 15 žen a 15 mužů. Vyšetřovací protokol se skládal z nutričního screeningu, bioelektrické impedanční analýzy (BIA), vyšetření síly stisku s pomocí ručního dynamometru a 30 s testu vstávání ze židle. Stav výživy byl dále posouzen prostřednictvím diagnostických kritérií GLIM a dotazníku Subjective Global Assessment (SGA). Výsledky vyšetření BIA, síly stisku a 30 s testu vstávání ze židle byly statisticky porovnávány pomocí nepárových testů - Welchova t-testu nebo Mann- Whitneyho U testu. Introduction: Huntington's disease (HD) is an inherited neuropsychiatric disorder with a fatal course. It is manifested mainly by motor symptoms and psychological changes. HD, like other neurological diseases, often results in the development of nutritional disorders due to many causes. Aims: The first aim of our research was to perform nutritional screening and examinate the nutritional status of a cohort of patients with HD and to determine in which parameters they differ from the healthy population. The second objective was to evaluate the sensitivity and specificity of the Malnutrition Universal Screening Tool (MUST) and the Global Leadership Initiative on Malnutrion (GLIM) in diagnosing malnutrition in HD patients. Methods: The patient and control cohort both consisted of 30 subjects, 15 women and 15 men. The examination protocol consisted of nutritional screening, bioelectrical impedance analysis (BIA), grip strength testing using a handheld dynamometer, and 30 second chair-stand test.
