Problematika náročného odpojování dlouhodobě ventilodependentního dospělého pacienta

Abstract

v ČJ Hlavním důvodem zvolení tohoto tématu je neustále narůstající počet dlouhodobě ventilodependentních pacientů, kteří podstupují odpojování od ventilační podpory na jednotkách intenzivní péče. V kapitolách je zmíněna anatomie a fyziologie dýchacího systému, patofyziologie a některá onemocnění spojována s ventilodependencí, vývoj a charakteristika oddělení OCHRIP - NIP a DIOP, postup odpojování a role sester v procesu odpojování. Ve výzkumném šetření jsou uvedeny cíle a hypotézy výzkumu a použitá metodika. Cílem výzkumného šetření bylo zmapovat zkušenosti a znalosti sester při odpojování pacientů od umělé plicní ventilace a druhým cílem bylo zjistit, jak sestry vnímají péči o tyto pacienty a jejich pohled na možnosti dalšího rozvoje této péče. Výzkumné šetření probíhalo na lůžkových jednotkách (ARO, ICU, NIP a DIOP) pěti nemocnic, kde stěžejní metodou bylo anonymní dotazníkové šetření, určené 147 respondentům (sestrám) na těchto odděleních a studentům dvou fakult v oboru Intenzivní péče. Hlavními výsledky našeho výzkumného šetření jsou, že sestry mají dostatečné znalosti a zkušenosti s odpojováním pacientů od ventilátoru, i když ony samy se podhodnocují, ale jejich praktické zkušenosti vyplývají pouze ze zvyklostí jejich pracoviště. The main reason for choosing the topic of study, is an increasing number of long-term ventilator-dependent patients undergoing weaning proces from artificial pulmonary ventilation at intensive care units. The content of the chapters include anatomy and physiology of the respiratory system, pathophysiology and some diseases associated with the ventilator-dependence. Also, the development and charakteristics of the OCHRIP - NIP and DIOP, the weaning procedure and the role of the nurses in the weaning proces. This study presents goals and hypothesis of investigation, as well as a variety of methodologies used. The study presents two aims of the research. Firstly, to map the experience and knowledge of nurses when disconnecting patients from artificial lung ventilation. Secondly, to find out how nurses perceive the care of these patients, including their view on the possibilities for further development of weaning process care. The research was carried out at ARO, ICU, NIP and DIOP bed units at five hospitals and two faculties of Intensive Care.
