Flexibilní dávkování inzulinu a dieta

Abstract

Tato bakalářská práce se zabývá vztahem mezi inzulinem a sacharidy v rámci metabolismu a nejvýznamnější poruchou těchto fyziologických pochodů ve formě onemocnění diabetes mellitus. Zabývá se také možnostmi léčby inzulínem u pacientů s diabetes mellitus 2. typu s důrazem na nutriční aspekt. Teoretická část je rozdělena na tři části. První představuje popis metabolismu sacharidů u zdravého člověka a představení inzulinu jako nezbytnou látku v tomto procesu. Druhá část je věnována patofyziologickému obrazu onemocnění, jeho příčinám a rizikovým faktorům. Podrobně jsou zde také popsány dva základní druhy nemoci - 1. a 2. typ. Třetí část je věnována léčbě. Řeší se zde možnosti nutriční terapie, ale také farmakoterapie inzulinem a jinými léčivy. Poslední část teoretické části vyúsťuje výzkumnou prací, jež se zabývá vlivem edukace a změny v inzulinoterapii z fixního na flexibilní dávkování u pacientů s diabetem mellitus 2. typu. Pacienti byli sledování po dobu šesti týdnů a byli během této doby opakovaně edukováni. K relevantnímu vyhodnocení byly použity změny hodnot glykovaného hemoglobinu a záznamy z glukometrů. Provedeným výzkumem bylo zjištěno na kolik má tato změna potenciál zlepšit kompenzaci diabetes mellitus a zda jsou pacienti dostatečně motivováni si nový princip dávkování osvojit. Sledovaní...

This bachelor thesis describes the connection between insulin and carbohydrates in the context of metabolism and the most significant disorder of these physiological processes in form of diabetes mellitus. It also discusses insulin treatment options with an emphasis on the nutritional aspect. The theoretical part is divided into three complex units. The first represents description of carbohydrate metabolism in a healthy person and introduction of insulin as an essential substance in this process. The second part is devoted to the representation of pathophysiological disorder, its causes and risk factors. There are also in detail described two basic types of the disease - type 1 and 2. The third part provides information about treatment. It focuses on possibilities of nutritional and pharmacological therapy. The last part of the theoretical part leads to research work, which examines the influence of education and change of insulin therapy from fixed to flexible dosing in patients with type 2 diabetes mellitus. Patients were observed for six weeks and during this time they were repeatedly educated.
