Wisconsinský test třídění karet. Výkon u zdravé české populace a pacientů se schizofrenií

Abstract

Wisconsinský test třídění karet (WCST)1 je obecně používán jako test exekutivních funkcí. Někteří autoři jej také nazývají testem přesunu myšlenkových setů, inhibice, flexibility, řešení problémů nebo kategorizace (Miyake a spol., 2000). Původně byl test vyvinut jako metoda měření schopnosti abstraktního myšlení a schopnosti přesouvat kognitivní strategie v reakci na nepředvídané změny v prostředí (Berg, 1948; Grant a Berg, 1948). Přestože byl WCST vyvinut a zpočátku používán u zdravé dospělé populace, stále více se prosazoval v klinické praxi jako nástroj neuropsychologické diagnostiky (Lezak, 1995; Spreen a Strauss, 1991). Během let používání se test vyvíjel a vznikaly jeho různé verze. V rigorózní práci se věnujeme Heatonově verzi testu (Heaton, 1981). Heaton vypracoval standardní systém administrace a skórování a dodal tak testu potřebnou exaktnost. Díky přesnosti hodnocení a množství objektivních skórů, které poskytuje, je Heatonova verze testu velmi oblíbená mezi výzkumníky. Populal'itn testu v kli11ické praxi prameni z určité senzitivity k poškozeni mozku, a především frontálních laloků, referované různými autory. (Drewe, 1974; Miller, 1963; Weinberger, Berman a Zec, 1986). Studium pacientů s různým poškozením mozku postupně dospělo k úvaze, že exekutivní funkce úzce souvisí s frontálními laloky....

Wisconsin Card Sorting Test (WCST) has been published as a measure of flexibility of thinking. Soon, it became a popular neuropsychological tool sensitive to brain damage, especially frontallobes. Arising from the knowledge of relationship between frontal lobes and executive functions, WCST is also called an executive task. Various versions of the test have been developed. Heaton' s standardization is in the focus of this thesis because it is the most often used version. Wisconsin Card Sorting Test is supposed to be the most common executive measure in neuropsychological research. The test output gives us 16 various scores, which reflect different cognitive processes. Each score has been provided with normative data on American population. However, clinically the most valid scores are scores of perseveration: perseverative responses and perseverative errors. There is certain evidence of impaired performance in patients with frontal lobe damage, especially left-side. Additionally, assumptions arising from physiological studies on healthy people or patients with various types of neurological illnesses suggest WCST to be sensitive not only to impairment of prefrontal cortex but also to the impairment of prefrontalsubcortical circ.uit including thalamus and basal ganglia or the prefrnntalsu bt:orli ca 1 t:l...
