Souhrn Nosní obstrukce je jedním z nejčastějších symptomů, se kterým se setkává otorinolaryngolog v klinické ambulanci. Etiologie nosní obstrukce je různorodá a určuje možnosti léčebního ovlivnění. V úvodní části práce je provedena analýza faktorů ovlivňujících nosní průchodnost a jsou uvedeny diagnostické možnosti umožňující posouzení, kvantifikaci i určení příčiny nosní neprůchodnosti. V patogenezi hraje roli statická a dynamická složka nosní neprůchodnosti. Dají se od sebe odlišit provedením dekongesčního testu. Jeho hodnocení pomocí RMM a AR je součástí klinické části práce. Dalším z cílů práce bylo vypracování standardního diagnostického protokolu pro nemocné s nosní obstrukci. V diagnostice existují diskrepance v názorech, která z diagnostických metod nejlépe a nejpřesněji odráží stupeň nosní obstrukce. Zjistili jsme, že subjektivní hodnocení nosní neprůchodnosti pacientem (dotazník), endoskopický nález a výsledky funkčných vyšetření navzájem ne zcela korelují. Příčinou je, že každá z těchto metod sleduje jiný parametr. Jde o metody vzájemně se doplňující, ne vzájemně se nahrazující. Proto se standardní diagnostický protokol skládá z podrobného anamnestického vyšetření nemocného (dotazník na základě VAS), rinoendoskopie, vyšetření RMM a AR včetně dekongesčního testu. Nasal obstruction is one of the most common problems the otorhinolaryngologist deals with at his office. Etiology of the nasal obstruction is various and determinates the treatment possibilities. Analysis of the factors influencing nasal patency is mentioned in the preliminary part of the work. The nasal obstruction is determined by statical and dynamical factors. The two factors can be differentiated by a decongestant test. The evaluation of the test by means of RMM and AR is presented in the clinical part of the work. Another aim of the study was to work out a standardised diagnostical protocol for patients with nasal obstruction. There are certains discrepancies within diagnostics as to which diagnostical method most accurately reflects the degree of nasal obstruction. What we found out in our study is, that subjective and objective measurements of the nasal patency and endoscopical findings do not always correlate. One of the reasons is that each of these methods is focusing on different aspects of the nasal airway. These are complementary methods, not supplying.
