Komunitní terapie poruch osobnosti

Abstract

Předkládaná práce si klade za cíl představit terapeutické komunity pro poruchy osobnosti. Ačkoli se u nás jedná o nový model terapie závažné osobnostní psychopatologie, jeho hlavní principy byly formulovány již před 60 lety britským psychiatrem M. Jonesem a od té doby jsou rozvíjeny v desítkách terapeutických komunit po celém světě. V první části jsou shrnuty současné poznatky o poruchách osobnosti jako takových. Důraz je přitom kladen na nastínění různých psychologických teorií poruch osobnosti, jejichž poznatky se nejčastěji uplatňují v terapeutické praxi. Je konstatováno, že tyto přístupy si vzájemně neodporují a nepopírají se a je proto užitečné chápat jejich přínos komplementárně. Dále jsou vyzdvihnuty některé specifické problémy spojené s psychoterapií poruch osobnosti a jejím empirickým výzkumem. Konceptu demokratické terapeutické komunity je věnována pozornost v další části práce. Jsou prezentovány jeho základní charakteristiky, které jej v mnohém odlišují od tradičního medicínského modelu: klienti se spolupodílejí na vedení komunity, je kladen důraz na aktivní roli klientů v terapii vlastní i druhých a na přijetí zodpovědnosti za svá rozhodnutí. The purpose of this thesis is to present therapeutic community treatment of personality disorders. Although in the Czech Republic this model represents a new therapeutic approach to the serious personality psychopathology, its principles were formulated already some 60 years ago by the British psychiatrist M. Jones and are being developed in dozens communities all over the world. The first section of this work summarizes present knowledge in the field of personality disorders. The stress is laid on introducing the different psychological theories of personality disorders which are most often applied in the therapeutic practise. It is claimed that these different approaches do neither contradict nor deny each other. Hence it is useful to understand their contributions complementary. Then there are highlighted some of the specific problems related to the psychotherapy of personality disorders as well as to its empirical research. The concept of democratic therapeutic community represents the subject in the next section of the work.
