Vliv zevní radioterapie v oblasti krku na hlasový a řečový projev českého mluvčího

Abstract

Disertační práce se věnuje výzkumu vlivu zevní radioterapie v oblasti krku na hlasový a řečový projev českých mluvčích. Cílem práce bylo zjistit, zda vlivem radioterapie v oblasti krku dochází ke změnám hlasového a řečového projevu českých pacientů, zda jsou tyto změny reverzibilní a nereverzibilní a co má na jejich reverzibilitu vliv. Rešeršní část práce shrnuje dosavadní výzkum problematiky v českém a mezinárodním prostředí a představuje různé přístupy ve zkoumání dané problematiky. Ve výzkumné části jsem nejprve popsal zvolené metodologické postupy, definoval výzkumnou a kontrolní skupinu studie a následně uvedl sérii experimentů, kterými jsem u konkrétních parametrů zjišťoval změny způsobené vlivem radioterapie. Zaměřil jsem se na akustické analýzy spojené s parametry produkce řeči a na percepční testy, kterými jsem zjišťoval změny hlasu a řeči a kvality hlasu v rámci řečové percepce. Vlivem radioterapie jsem nejvýznamnější změny zjistil v hodnotách základní frekvence před a po radioterapii a v průběhu kontur základní frekvence, dále u vybraných parametrů spektrálního sklonu a u mluvního tempa. The doctoral thesis focuses to researching the effect of external radiotherapy in the neck area on the voice and speech of Czech speakers. The aim of the work was to determine whether radiotherapy in the neck area causes changes in the voice and speech of Czech patients, whether these changes are reversible or irreversible, and what influences their reversibility. The theoretical part of the work summarizes the current research on the issue in the Czech and international environment and presents various approaches to examining the issue. In the research part, I first described the selected methodological procedures, defined the research and control groups of this study, and then introduced a series of experiments with which I investigated changes caused by radiotherapy in specific parameters. I focused on acoustic analyses associated with speech production parameters and on perceptual tests, with which I used to find out changes in voice and speech and voice quality within speech perception. Due to radiotherapy, I found the most significant changes in the values of the fundamental frequency before and after radiotherapy and during the fundamental frequency contours, as well as in selected parameters of the spectral slope and speech tempo.
