human induced pluripotent stem cells, age-related macular degenerationVěkem podmíněná makulární degenerace (AMD) je multifaktoriální, progresivní onemocnění, které nyní postihuje více než 200 milionů lidí starších 60 let. Konečné stádium onemocnění se vyskytuje ve dvou formách, běžně označovaných jako "vlhká" a "suchá". "Vlhká" (neovaskulární) forma je charakterizována neovaskularizací a prosakováním tekutin mezi vrstvami sítnice. Tato forma je v současnosti léčena pomocí nitroočních injekcí anti-VEGF protilátky. "Suchá" forma se projevuje geografickou atrofií (GA), zejména v oblasti makuly. Ta má nejvyšší hustotu čípků a je nejvíce metabolicky aktivní. Tato forma je zatím považována za neléčitelnou. Současné možnosti léčby zahrnují změny životního stylu a potlačení chronického zánětu, což však pouze částečně zpomaluje progresi onemocnění. Výzkum po celém světě se proto zaměřuje na hledání bezpečné, efektivní a dostupné léčby. U pacientů s GA je hlavním cílem zastavení progrese atrofie a obnovení narušeného metabolismu. Naším přístupem je transplantace buněk pigmentového epitelu sítnice (RPE) do oblasti okraje GA s cílem zastavit postup atrofie, nahradit metabolické a fagocytární funkce a obnovit krevně-sítnicovou bariéru, která umožní správný transport živin a odpadních produktů. Age-related macular degeneration (AMD) is a multifactorial, progressive disease that affects over 200 million people over 60 years old. The end stage of this disease has two forms, colloquially known as <wet= and <dry=. <Wet= or neovascular form is characterised by neovascularisation and fluid leakage between the layers of the retina. It is currently treatable by anti-VEGF intraocular injections. Dry form is characterised in later stages by geographical atrophy (GA) mainly in the macular area. This area has the highest density of cones, and it is the most metabolically active. It is currently considered untreatable. Current options include diet and lifestyle changes, as well as the suppression of chronic inflammation, which can only partially slow down the progression. Research worldwide is focused on identifying safe, cost-effective, and efficient treatments. In patients with GA, the primary goal is to stop the progression and re-establish the disrupted metabolism. Therefore, we focus on transplantation of RPE cells to the edge of GA, which could stop the progression, replace metabolic and phagocytic activity, and establish a new blood-retinal barrier to ensure proper transport of debris and nutrients.
