Využití SPECT/CT vyšetření v operační léčbě degenerativního postižení páteře

Abstract

The presented dissertation primarily focuses on examining the use of SPECT/CT in the surgical treatment of degenerative spine disorders, including degeneration of intervertebral discs, adjacent vertebral endplates, vertebral bodies and intervertebral joints. The aim of the first part of the thesis was to assess and define the potential applications of SPECT/CT in the treatment of spine degeneration through a systematic review. The second part describes two studies. The first study is dedicated to the radiological evaluation and comparison of findings related to degenerative changes in the lumbar spine using magnetic resonance imaging (MRI) and SPECT/CT. The second study focuses on the outcomes of stabilization procedure performed at SPECT/CT - positive segments of the lumbar spine. The results of the studies demonstrate a strong association between SPECT/CT positivity and the severity of degenerative changes in the lumbar spine. The experimental study concludes with evidence of significant improvement in symptoms and patient satisfaction following single-segment fusion surgery performed at SPECT/CT - positive segments of the lumbar spine.

Předkládaná disertační práce se primárně zabývá zkoumáním využití SPECT/CT vyšetření v diagnostice a operační léčbě degenerativního postižení páteře jak postižení meziobratlové ploténky, krycích plotének obratlů a přilehlých částí obratlových těl, tak meziobratlových kloubů. Cílem první části práce bylo za pomoci systematického přehledu literatury zhodnotit a definovat možnosti využití SPECT/CT v léčbě degenerativního postižení páteře. Ve druhé části jsou popsány dvě studie. První je věnovaná radiologickému hodnocení a srovnání nálezů degenerativního postižení bederní páteře pomocí magnetické rezonance (MR) a SPECT/CT. Druhá studie se věnuje výsledkům stabilizačních výkonů provedených v úrovni SPECT/CT pozitivních segmentů bederní páteře. Z výsledků studií vyplývá asociace mezi SPECT/CT pozitivitou a tíží degenerativních změn v oblasti bederní páteře. Výsledkem experimentální klinické studie je průkaz signifikantního zlepšení obtíží a spokojenosti pacientů s jednosegmentovým stabilizačním výkonem provedeným ve SPECT/CT pozitivním segmentu bederní páteře.
