Medicínský diskurz v kontextu konstrukce genderové binarity

Abstract

This bachelor's thesis examines medical discourse in the context of gender binary construction. It focuses on analysing how contemporary medicine in the Czech Republic approaches issues of gender and sex, and how this discourse affects individuals who do not fit into traditional binary categorization, particularly intersex, non-binary, and transgender people. The research is based on qualitative semi-structured interviews with young doctors from various medical fields. The analysis shows that while doctors are aware of gender diversity and incorporate it into certain aspects of clinical practice, the dominant medical approach still adheres to a binary model of sex. This perspective is particularly evident in diagnostic protocols, treatment approaches, and medical education, where sex is often perceived as a fixed biological category. At the same time, respondents recognize gender in a broader sociocultural context and, in interpersonal interactions with patients, respect it as a fluid and individually defined identity. This multi-layered approach demonstrates that while medical discourse structurally reinforces the binary sex model, there is room for more gender-sensitive practices within communication and clinical care. Tato bakalářská práce se zabývá medicínským diskurzem v kontextu konstrukce genderové binarity a jeho vlivem na klinickou praxi, vzdělávání a zkušenosti lékařů a lékařek v České republice. Zaměřuje se na analýzu toho, jak současná medicína přistupuje k otázkám genderu a pohlaví a jak tento diskurz ovlivňuje osoby, které nespadají do tradiční binární kategorizace, zejména intersex, nebinární a trans osoby. Výzkum je založen na kvalitativních rozhovorech s mladými lékaři a lékařkami různých specializací. Analýza ukazuje, že ačkoliv jsou si lékaři a lékařky vědomi genderové diverzity a v některých aspektech ji reflektují, dominantní medicínský přístup nadále operuje s binárním modelem pohlaví. Tento přístup se projevuje zejména v diagnostických postupech, přístupu k léčbě a ve vzdělávacím systému, kde je pohlaví často vnímáno jako pouze biologická kategorie. Současně však respondenti a respondentky vnímají gender v širším sociokulturním kontextu a v rámci mezilidské interakce s pacientstvem jej respektují jako proměnlivou a individuálně definovanou identitu. Tato vrstevnatost v přístupu naznačuje mírný rozpor v medicínském diskurzu a zároveň poukazuje na potenciální institucionální bariéry.
