Používání informovaného souhlasu na jednotce intenzivní péče

Abstract

The author talks in this assignment about inform consent used in acute cardiology. Firstly the explanation is used to show the main differences between the information given out to patients prior and after taking Convention for Protection of Human Rights and dignity of the Human Being with regard to the Aplication of Biology and Medicine. Then it points out the reasons behind why the informed consent was needed, its forms and related legalization. After that the theoretical and practical problems in use of this consent are discussed. The methods used in the practical part are quantitative and qualitative such as questionnaires, patients' interviews on intensive units and cardiac ward and content analysis. The outcome from the information obtained is that the consent is not used correctly in practice. Also those patients expect the consultant to make the decision for them and they do not understand the significance of informed consent. Written consent does not match the expectation with regulation vyhláška č. 64/2007 Sb., o zdravotnické dokumentaci at all. Also they are too long, contains terminology which are not used by patients which leads to a misunderstanding. On the basis of these findings the reviews and recommendations are suggested.

Autorka se v diplomové práci zabývá používáním informovaného souhlasu v akutní kardiologii. Je popsán způsob informování pacientů před a po přijetí Konvence na ochranu lidských práv a důstojnosti lidské bytosti v souvislosti s aplikací biologie a medicíny. V dalších částech práce popisuje důvody vzniku informovaného souhlasu, jeho formy i zákonné normy, které používání informovaného souhlasu upravují. Nastiňuje teoretické i praktické problémy používání informovaného souhlasu v oboru kardiologie. V praktické části používá metod kvantitativního a kvalitativního výzkumu (dotazníkové šetření, rozhovory s pacienty jednotky intenzivní péče a standardního kardiologického oddělení, obsahová analýza). Z výsledků šetření vyplývá skutečnost, že institut informovaného souhlasu není v praxi správně a důsledně používán. Pacienti očekávají paternalistický přístup ze strany lékařů, většinou si neuvědomují závažnost aktu projevení své vůle informovaným souhlasem. Písemné souhlasy nesplňují formální a obsahové požadavky vyhlášky č. 64 /2007 Sb., o zdravotnické dokumentaci, navíc jsou dlouhé, obsahují velké množství cizích slov a tudíž jsou pro pacienty obtížně srozumitelné. Na základě takto získaných poznatků navrhuje možné cesty ke změně.
