Úvod: Zdravotnický záchranář (ZZ) v přednemocniční neodkladné péči nemůže na místě zásahu sám, bez konzultace s lékařem, indikovat podání analgetik. Z tohoto důvodu může docházet k časovým prodlevám, které prodlužují utrpení a bolest pacienta s akutním traumatem. Cíle: Prvním cílem je hodnocení subjektivního vnímání léčby bolesti ZZ a zjištění ochoty ZZ přijmout kompetenci k podávání sufentanilu bez konzultace lékaře. Druhým cílem je vytvoření a implementace vzdělávacího programu pro zisk této kompetence. Třetím cílem je zhodnocení výskytu komplikací intravenózně podávaného sufentanilu samostatně ZZ. Metodika: První cíl je realizován formou dotazníkového šetření mezi ZZ (n=118) Zdravotnické záchranné služby Karlovarského kraje. Druhý cíl je dosahován tvorbou a implementací vzdělávacího programu pro získání kompetence k podání sufentanilu u pacientů s akutním traumatem. Třetím cílem je hodnocení výskytu komplikací podávání sufentanilu během 6měsíčního období porovnáním dvou skupin intravenózního podání sufentanilu ZZ u dospělých pacientů s akutním traumatem: 1) kompetence (n = 70) samostatně na základě vzniklé kompetence; 2) konzultace (n = 88) po telefonické konzultaci s lékařem. Introduction: A paramedic in pre-hospital emergency care cannot indicate the administration of analgesics alone at the site of the intervention, without consulting a doctor. For this reason, there may be time delays that prolong the patient's suffering and pain and acute trauma. Aims: The first aim is to evaluate the paramedics subjective perception of the pain treatment and to determine their willingness to accept the competence to administer sufentanil without consultation. The second goal is to create and implement an educational program to gain this competence. The third goal is to evaluate the incidence of complications of sufentanil administered without doctor cosultation. Methodology: The first goal is realized in the form of a questionnaire survey among the paramedics (n = 118) at the Emergency Medical Service of the Karlovy Vary Region.
