Vliv kondice pacientů před transplantací plic na délku hospitalizace po transplantaci

Abstract

This thesis aims to assess whether the level of the physical condition of patients before lung transplantation affects the length of hospitalization after transplantation. The study includes patients over 18 years of age with both unilateral and bilateral lung transplantation who underwent hospitalization between 2016 and 2018 and underwent a six-minute walk test in the preoperative examination. Data were collected at the 3rd Department of Surgery, First Faculty of Medicine, Charles University in Prague and Motol University Hospital and at the Department of Pneumology, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital. The results partially confirmed the correlation between hospitalization and physical condition in some patient groups. Thus, the higher the level of fitness in patients before lung transplantation, the shorter the hospitalization period. The length of hospitalization is influenced not only by the condition of the patient before surgery but also by other significant preoperative, perioperative and postoperative factors that are not related to the condition. Tato diplomová práce si klade za cíl posoudit, zda má úroveň kondice u pacientů před transplantací plic vliv na délku hospitalizace po transplantaci. Byly vyhodnocovány údaje pacientů starších více jak 18 let jak s jednostrannou, tak i s oboustrannou transplantací plic, kteří prošli hospitalizací v období mezi rokem 2016 a 2018 a v předoperačním vyšetření podstoupili šestiminutový test chůzí. Sběr dat probíhal na 3. chirurgické klinice 1. LF a FN Motol a Pneumologické klinice 2. LF a FN Motol. Výsledky částečně potvrdily nepřímou úměru mezi délkou hospitalizace a úrovní kondice u některých skupin pacientů. Tedy čím vyšší byla úroveň kondice u pacientů před transplantací plic, tím kratší byla délka hospitalizace. Délka hospitalizace je ovlivněna nejen kondicí pacienta před operací, ale i dalšími významnými předoperačními, perioperačními i postoperačními faktory, které s kondicí nesouvisí. Z tohoto důvodu nelze obecně vyvodit jasnou korelaci mezi délkou hospitalizace a kondicí. Zlepšení kondice před očekávanou operací je žádoucí u kandidátů na transplantaci plic, tak aby se podpořila úspěšnost transplantace plic.
