Abstract

Rybníčková Soňa MOŽNOSTI STANOVENÍ PROTILÁTEK TYPU LUPUS ANTIKOAGULANS V HEMATOLOGICKÉ LABORATOŘI IKEM PRAHA Bakalářská práce Univerzita Karlova v Praze, Farmaceutická fakulta v Hradci Králové Zdravotní laborant Abstrakt Cíl práce Diagnostika lupusu antikoagulans je problematická, protože neexistuje 100% citlivý screeningový test, a v případě prodloužení koagulačních testů je nutno vyloučit i jiné možné příčiny. (M. Penka 2011) Cílem mé práce je ukázat možnosti vyšetřování LA v naší laboratoři, někdy nejednoznačnou interpretaci naměřených výsledků a také nutnost dodržovat preanalytickou fázi. Zhodnotit klinické projevy, laboratorní kritéria, popsat možné interference testů LA. Metody Teoretická část se zabývá základními mechanismy hemostázy, působením protilátek typu LA v organismu, diagnostickými kritérii i léčebnými doporučeními. Praktická část ukazuje, i pomocí jednoduchého centrifugačního pokusu, jak je důležitá preanalytická fáze včetně dvojí centrifugace, popisuje použité reagencie a přesný postup vyšetření LA rozdělený na screeningové, směsné a konfirmační testy. Výsledky Vyhodnocujeme výsledky jednotlivých testů: screeningových (aPTT-LA a dRVVT screen), směsných testů (aPTT-LA A dRVVT) a konfirmačních testů (ACTIN FS a dRVVT confirm, Staclot® LA). Rybníčková Soňa THE POSSIBILITY OF ASSESSMENT OF LUPUS ANTICOAGULANT ANTIBODIES IN HEMATOLOGY LABORATORY IKEM IN PRAGUE Bachelor thesis Charles University in Prague, Faculty of Pharmacy in Hradec Králové Laboratory Technician Background Diagnosis of the Lupus Anticoagulant is very complicated due to a missing completely reliable screening test and whenever the presence of prolonged clotting times on a routine plasma test is detected, the other potential causes besides LA is needed to exclude. The aim of the thesis is focused on a possibilities of testing the Lupus Anticoagulant in our Clinical Haematology Department in the Institute for Clinical and Experimental Medicine inclusive of ambiguity measured results interpretation and a necessity to abide by the conditions of a preanalytic phase. In addition, clinical manifestation and laboratory criteria assessment as well as a possible interferences in Lupus Anticoagulant testing are also discussed. Methods The theoretical part is focused on a basic mechanism of haemostasis, on an effect of antiphospholipid antibodies, the diagnostic kriteria inclusive of a treatment guidelines.
