Problematika ošetřovatelské péče u pacientů s neinvazivní plicní ventilací

Abstract

Východiska: Bakalářská práce je zaměřena na problematiku ošetřovatelské péče u dětských pacientů s neinvazivní plicní ventilací (NIV). Teoretická část popisuje principy NIV, indikace, kontraindikace a značná část je věnována specifickým komplikacím. Zároveň se zabývá rolí sestry v péči o pacienta s tímto typem dechové podpory. Cíle: Hlavním cílem práce bylo zhodnotit teoretické i praktické znalosti a zkušenosti sester v oblasti neinvazivní plicní ventilace, a identifikovat nejfrekventovanější komplikace. Metodika: Kvantitativní šetření, proběhlo ve Fakultní nemocnici Motol a Homolka. Byl využit dotazník vlastní konstrukce, který obsahoval celkem 19 otázek. Konečný výzkumný soubor tvoří 74 respondentů. Výsledky: Výsledky reflektují dobré znalosti respondentů v praktických ošetřovatelských postupech a monitoringu. Zároveň prokázaly deficit znalostí kolidující s aktuálními doporučeními v odborné literatuře. Bylo zjištěno, že délka praxe sester statisticky významně ovlivnila řešení abdominální distenze (p= 0,030) a způsob kontroly fixace rozhraní (p=0,032). Závěr: Na základě zjištěných výsledků jsou v závěru práce formulována konkrétní doporučení pro ošetřovatelskou praxi.

Background: This bachelor's thesis focuses on nursing care issues for pediatric patients undergoing non-invasive ventilation (NIV). The theoretical section describes the principles, indications, and contraindications of NIV, with a significant portion devoted to specific complications. It also addresses the role of the nurse in caring for patients receiving this type of respiratory support. Objectives: The primary objective of the thesis was to evaluate the theoretical and practical knowledge and experience of nurses in the field of non-invasive ventilation and to identify the most frequent complications. Methodology: A quantitative study was conducted at Motol University Hospital and Na Homolce Hospital. A self-designed questionnaire consisting of 19 questions was utilized. A total of 74 completed questionnaires were collected from pediatric inpatient departments. Results: The results reflect good knowledge among the respondents regarding practical nursing procedures and monitoring. At the same time, they revealed a knowledge deficit that conflicts with current recommendations in the professional literature.
