Abstract

This bachelor thesis deals with nursing care of the patient with chronic kidney disease based on IgA nephropathy who has had a kidney transplant from a cadaverous donor, when less than 3 weeks after the transplant there was a complication of renal vein thrombosis and subsequent graft rejection. The aim of this work is to describe, analyse and evaluate nursing care in relation to complications in a kidney transplant patient. The bachelor thesis consists of two parts, namely the theoretical part and practical part. The theoretical part mentions anatomy and kidney physiology and also renal failure. Mostly, there is a focus on chronic renal disease and subsequent kidney transplantation. These two chapters are discussed in more detail because of the link between the patient's disease and subsequent kidney transplant she underwent. There is also focus on post- transplant complications, as the patient lost the transplant kidney due to one of the complications. The practical part describes the post - operative care of the patient after a kidney transplant in nephrology department. In this case, this case study deals with the description of the patient's medical condition on day 3-15 of the post - op day, it follows comprehensively this hospitalization and the patient's condition after discharge. Tato bakalářská práce se zabývá ošetřovatelskou péčí o pacientku s chronickým onemocněním ledvin na podkladě IgA nefropatie, u které proběhla transplantace ledviny od kadaverózního dárce, kdy necelé tři týdny po transplantaci došlo ke komplikaci v podobě trombózy renální žíly a následné rejekci štěpu. Cíl této práce je zaměřen na popis, analýzu a zhodnocení ošetřovatelské péče v souvislosti s komplikacemi u pacientky po transplantaci ledviny. Bakalářská práce se skládá ze dvou částí, konkrétně z teoretické části a praktické části. V teoretické části je zmíněna anatomie a fyziologie ledvin a také renální selhání. Nejvíce je zde zaměření na chronické renální onemocnění a následnou transplantaci ledviny. Tyto dvě kapitoly jsou rozebírány podrobněji z důvodu souvislosti mezi pacientčiným onemocněním a následnou transplantací ledviny, kterou podstoupila. Zaměření je zde také na komplikace po transplantaci, jelikož pacientka o transplantovanou ledvinu přišla z důvodu jedné z komplikací. V praktické části je popisována pooperační péče o pacientku po transplantaci na lůžkovém oddělení kliniky nefrologie. V tomto případě se kazuistika zaobírá popisem zdravotního stavu pacientky v 3. až 15. pooperačním dni, sleduje komplexně tuto hospitalizaci a stav pacientky po propuštění.
