Standardy intratekální antibiotické terapie a role sestry při její realizaci

Abstract

This master's thesis examines the issue of intrathecal antibiotic administration from the perspective of non-medical healthcare professionals (NHP). This specific route of administration is indicated for severe infectious diseases of the central nervous system in cases where systemic therapy is insufficient or ineffective. The theoretical part of the thesis provides a comprehensive overview of the physiology of the blood-brain and blood-cerebrospinal fluid barriers and details the specifics of intrathecal administration. The text further discusses various access methods to the cerebrospinal fluid space, the selection of appropriate antibiotics, and the associated legislative and safety requirements. It also includes an analysis of potential complications and defines adequate clinical responses to their occurrence. The research part is based on a survey conducted using anonymous non-standardized questionnaires. Data collection took place in the Neurosurgical Intensive Care Unit, the Semi- intensive Neurosurgical Unit, the Neurological ICU, and the Department of Anesthesiology and Resuscitation (ARO). Tato diplomová práce se věnuje problematice intratekální aplikace antibiotik z perspektivy nelékařského zdravotnického personálu (NLZP). Tento specifický způsob podání léčiv je indikován u závažných infekčních onemocnění centrálního nervového systému v případech, kdy je systémová terapie nedostatečná či neúčinná. Teoretická část práce podává ucelený přehled o fyziologii hematoencefalické a hematolikvorové bariéry a podrobně charakterizuje specifika intratekální aplikace. Text se dále zabývá popisem možných přístupů do mozkomíšního prostoru, výběrem vhodných antibiotik a legislativními i bezpečnostními požadavky na tento výkon. Neopomíjí ani analýzu potenciálních komplikací a definování adekvátních reakcí na jejich výskyt. Výzkumná část vychází z dotazníkového šetření, které bylo realizováno formou anonymních standardizovaných dotazníků. Sběr dat probíhal na neurochirurgické jednotce intenzivní péče, semiintenzivní neurochirurgické jednotce, neurologické JIP a na oddělení anesteziologie a resuscitace (ARO). Šetření bylo zaměřeno na zmapování znalostí a klinických postupů u všeobecných sester, praktických sester a zdravotnických záchranářů se zvláštním zřetelem na proces přípravy, samotnou aplikaci a následnou monitoraci pacienta.
