fluid stewardship, ICU infusionIntravenózní tekutiny představují důležitý nástroj a jednu z nejčastěji využívaných intervencí v léčbě kriticky nemocných pacientů. Současné poznatky však upozorňují na rizika spojená s jejich neuvážlivým podáváním, které může vést k rozvoji závažných komplikací v podobě tekutinového přetížení. V reakci na přítomná rizika tekutinové terapie vznikl koncept tekutinového stewardshipu, který zdůrazňuje význam šetrného zacházení s intravenózními tekutinami. Cílem této práce bylo shrnout aktuální poznatky v oblasti tekutinového managementu kriticky nemocných pacientů a zhodnotit vývoj tekutinové praxe pomocí komparativní analýzy. Empirická část představuje prospektivní kohortovou studii s historickou kontrolou. Jedná se o observační design zaměřený na sběr dat, který byl realizován na Klinice anesteziologie a resuscitace ve Fakultní nemocnici Královské Vinohrady. Zkoumaný soubor tvořily dvě kohorty pacientů z roku 2014 a 2024. Výsledky komparativní analýzy poukázaly na změnu tekutinové terapie směrem k šetrnější strategii v roce 2024. U prospektivní kohorty byly zaznamenány nižší objemy resuscitačních i udržovacích tekutin, zatímco u retrospektivní kohorty z roku 2014 byla tekutinová terapie intenzivnější. Intravenous fluids represent an important therapeutic tool and one of the most frequently used interventions in the treatment of critically ill patients. Current evidence, however, highlights the risks associated with their imprudent administration, which may lead to the development of serious complications in the form of fluid overload. In response to the existing risks of fluid therapy, the concept of fluid stewardship has emerged, emphasizing the importance of careful and rational use of intravenous fluids. The aim of this thesis was to summarize current knowledge in the field of fluid management in critically ill patients and to evaluate the development of fluid therapy practices through comparative analysis. The empirical part represents a prospective cohort study with a historical control. It is an observational design focused on data collection, conducted at the Department of Anesthesiology and Resuscitation of the University Hospital Královské Vinohrady.
