Komunikace s pacientem v závěru života a jeho blízkými z pohledu sester intenzivní péče a sester paliativní péče

Abstract

This thesis focuses on how nurses working in intensive care perceive and experience communication with dying patients and their loved ones. Communication is an integral part of nursing practice; however, in certain situations it can be particularly challenging. The aim of this thesis is to explore how nurses perceive communication with patients at the end of life and their relatives, and to develop recommendations for intensive care nurses on communicating with end-of-life patients and their loved ones, including a summary of the communication competencies nurses possess. Based on interviews with intensive care nurses, the study seeks to identify possible barriers and limitations in communication with end-of-life patients and their relatives. Through interviews with palliative care nurses-who engage in this type of communication on a daily basis-the thesis aims to identify approaches and strategies that intensive care nurses can adopt based on their experience. This is a qualitative study conducted through semi-structured interviews, involving 6 respondents from intensive care and 4 from palliative care. Tato práce se zaměřuje na to, jak sestry pracující v intenzivní péči vnímají a prožívají komunikaci s umírajícími pacienty a jejich blízkými. Rozhovor je nedílnou součástí práce sester. V některých situacích ale může být komunikace opravdu náročná. Cílem této práce je zjistit, jak sestry vnímají komunikaci s pacientem v závěru života a jeho blízkými a vytvořit doporučení pro sestry v intenzivní péči v komunikaci s pacienty v závěru života, či jejich blízkými, včetně souhrnu komunikačních kompetencí, které sestry mají. Na základě rozhovorů se sestrami v intenzivní péči chci zjistit možné bariéry a limity v komunikaci s pacienty v závěru života, či jejich blízkými. Pomocí rozhovorů se sestrami v paliativní péči, pro které je tento typ komunikací každodenní činnost, chci najít možnosti a postupy, které z jejich zkušeností mohou sestry v intenzivní péči využít. Jedná se o kvalitativní výzkum provedený polostrukturovanými rozhovory, kterého se zúčastnilo 6 respondentů z intenzivní péče a 4 z paliativní péče. První, teoretická část vymezuje pojmy "intenzivní péče" a "terminální pacient". Dále se věnuje komunikaci a komunikačním kompetencím sester. Druhá, praktická část se zabývá metodologií kvalitativního výzkumu za použití polostrukturovaného rozhovoru. Seznamuje s cíli a s výzkumnými otázkami.
