Reliabilita sonografického měření mezižeberních prostor

Abstract

This thesis aimed to explore the reliability of sonography measurement of the intercostal spaces, shared between two therapists. The theoretical part summarised findings related to ontogenesis, anatomy and function of the thorax and respiratory system, kinematics of the thorax and technical and clinical aspects of the locomotor system sonography. Methodology: 20 male participants between the ages of 20 and 30 without any health restrictions took part in the study. Sonography measurements were performed by two therapists in two consecutive repetitions. The measurements were performed during the participant's maximal inspiration and passive exhalation in the 5th-6th intercostal space with the body positioned on the back and on the side, each time on the left and right side of the body. The breathing was controlled by the POWERbreathe K5 device, while the Alpinion X- CUBE i8 sonography device was used for the measurement itself. The data were then processed using descriptive statistics. Results: Good reliability was demonstrated in the measurements of both therapists (ICC > 0.6). The results also demonstrate the dependence of the body position on the enlargement of the intercostal spaces during inspiration. Tato diplomová práce byla zaměřena na reliabilitu sonografického měření mezižeberních prostor mezi dvěma terapeuty. V teoretické části byly shrnuty poznatky v souvislosti s ontogenezí, anatomií a funkcí hrudníku a dýchacího systému, kinematiky hrudníku a technických i klinických aspektů sonografie hybného systému. Metodika: Do výzkumu bylo zapojeno 20 jedinců mužského pohlaví bez zdravotního omezení ve věkovém rozmezí 20-30 let. Sonografické měření bylo provedeno dvěma terapeuty ve dvou po sobě jdoucích opakováních. Měření probíhalo při maximálním nádechu a klidovém výdechu probanda v lokalitě 5.-6. mezižeberního prostoru v pozici těla na zádech a na boku, vždy na levé i pravé straně těla. Kontrola nádechu byla provedena pomocí přístroje POWERbreathe K5 a na samotné měření byl využit sonografický přístroj Alpinion X-CUBE i8. Výsledky byly následně zpracovány pomocí deskriptivní statistiky. Výsledky: Byla prokázána dobrá reliabilita v měření obou terapeutů (ICC > 0,6), výsledky také potvrzují závislost polohy těla na zvětšování mezižeberních prostor při nádechu. Závěr: Výsledky práce potvrzují dobrou reliabilitu v sonografickém měření mezižeberních prostor.
