Screening vestibulárních funkcí a poruch okulomotoriky u geriatrických pacientů

Abstract

Dysfunction of the vestibular system and oculomotor disorders can contribute to the risk of falls in geriatric patients, affecting their daily activities and reducing their quality of life. The aim of this thesis was to determine the prevalence of vestibular dysfunction and oculomotor disorders in seniors hospitalized at the Geriatric Internal Medicine Clinic of the Second Faculty of Medicine, Charles University, and Motol University Hospital, and at the Motol University Hospital Long-Term Care Department. We also investigated whether patients with benign paroxysmal positional vertigo show a higher frequency of abnormalities in vestibular and oculomotor function tests compared to patients without this diagnosis. A total of 103 patients were examined. Information on the occurrence of falls was obtained from medical history. Vestibular functions were assessed using the Head Impulse Test, Subjective Visual Vertical and the presence of nystagmus. Smooth pursuit and saccadic eye movements and vestibulo-ocular reflex supression were examined from oculomotor functions. Dysfunkce vestibulárního systému a poruchy okulomotoriky mohou u geriatrických pacientů přispívat k riziku pádu, ovlivnění každodenních činností a snížení kvality života. Cílem této práce bylo zjistit prevalenci poruch vestibulární funkce a poruch okulomotoriky u seniorů hospitalizovaných na Geriatrické interní klinice 2. LF UK a FN Motol a Centru následné péče FN Motol. Dále jsme zjišťovali, zda pacienti s benigním paroxysmálním polohovým vertigem vykazují častější přítomnost abnormit ve vyšetření vestibulárních a okulomotorických funkcí v porovnání s pacienty bez této diagnózy. Bylo vyšetřeno 103 pacientů. Informace o výskytu pádů byly získány anamnesticky. Vestibulární funkce byly hodnoceny pomocí Head Impulse Test, subjektivní zrakové vertikály a přítomnosti nystagmu. Z okulomotorických funkcí byly vyšetřeny plynulé a sakadické oční pohyby a suprese vestibulo-okulárního reflexu. Pád během posledních šesti měsíců uvedlo 54 % pacientů. Dále 54, 4 % pacientů mělo abnormitu alespoň jednoho klinického testu hodnotícího vestibulární funkce. Na 59 % pacientů mělo abnormální alespoň jedno z vyšetření okulomotoriky. Nebyly zjištěny statisticky významné rozdíly v incidenci pádů, dysfunkcích vestibulárního systému ani abnormitách okulomotoriky v porovnání pacientů s BPPV s pacienty bez BPPV. Tato práce...
