Onemocnění temporomandibulárního kloubu - lupavý kloub

Abstract

Tato práce se zabývá lupavým kloubem při onemocnění temporomandibulárního kloubu. Lupání v čelistním kloubu vzniká na podkladě dislokace disku s repozicí, adherence disku nebo subluxace mandibuly. V teoretické části jsou popsány jednotlivé typy lupání, biomechanika, kineziologie, funkční anatomie a patokineziologie temporomandibulárního kloubu. Práce se dále věnuje možnostem léčby lupavého čelistního kloubu, mezi které patří izometrické posilování a postizometrická relaxace musculus pterygoideus lateralis, manuální uvolňování spoušťových a citlivých bodů, okluzní dlahy, nízkovýkonný laser a další metody. Cílem praktické části bylo zpracování kazuistiky jednoho pacienta s příznakem lupání v čelistním kloubu. Po osmitýdenní terapii došlo ke snížení skóre v dotazníku funkčního omezení čelisti (JFLS-20) ze vstupních 18 bodů na 11 bodů, a při kontrolním hodnocení (měsíc po ukončení terapie) na 8 bodů. Subjektivně pacient vnímal pokles intenzity lupání na škále 0-3, kdy se intenzita lupání snížila z původní hodnoty 2 (způsobující diskomfort) na 1 (minimální lupání), a na vizuální analogové škále (VAS 10 cm), na které se snížila hodnota v průměru o 3 cm k poslední terapii a o 1,7 cm při kontrolním vyšetření. The thesis focuses on joint clicking in temporomandibular joint (TMJ) disorders. Clicking in the jaw joint occurs due to disc displacement with reduction, disc adherence, or mandibular subluxation. The thesis describes the types of clicking, the biomechanics, kinesiology and anatomy of the temporomandibular joint, as well as its pathokinesiology. The thesis also examines treatment methods for a clicking jaw joint, centered on isometric strengthening/post- isometric relaxation of the lateral pterygoid, manual trigger point release, occlusal splint use, and low-intensity laser applications. The aim of the thesis was a case study of a patient with clicking temporomandibular joint. After an eight-week therapy program, the patient's Jaw Functional Limitation Scale (JFLS-20) score decreased from an initial 18 points to 11 points, and further to 8 points at the follow-up assessment one month after the intervention. Subjectively, the patient reported a reduction in clicking intensity on a 0-3 scale, with values decreasing from 2 (causing discomfort) to 1 (minimal clicking). On the 10 cm visual analogue scale (VAS), the average score decreased by 3 cm by the end of therapy and by 1.7 cm at the follow-up evaluation.
