Hodnocení polykání u pacientů na oddělení Následné intenzivní neurorehabilitace - klinický doporučený postup

Abstract

This diploma thesis deals with the issue of dysphagia in patients in the department of Subsequent Intensive Neurorehabilitation. Patients with acquired neurodeficits are admitted to this type of workplace. These patients are at risk of silent pulmonary aspiration and at the same time often have their airways secured with a tracheostomy cannula, which in the topic of swallowing represents two major complicating factors. A serious complication of silent aspiration is aspiration pneumonia, which is associated with a high risk of morbidity and mortality. The theoretical part contains theoretical foundations of the physiology of swallowing, the pathophysiology of swallowing disorders with a focus on neurogenic dysphagia and neurological diseases that may affect swallowing. Furthermore, it includes the assessment of swallowing, the effect of tracheostomy on swallowing and decannulation of the tracheostomy cannula in the context of dysphagia. An important part is also the role of the nurse in the management of dysphagia and the possibilities of education nurses in this issue. The practical part is prepared in the form of a clinical recommendation, the result of which is a manual for nurses. Tato diplomová práce se zabývá problematikou dysfagie u pacientů na oddělení Následné intenzivní neurorehabilitace. Na pracoviště tohoto typu jsou přijímáni pacienti se získaným neurodeficitem. Tito pacienti jsou ohroženi rizikem tiché plicní aspirace a zároveň mají mnohdy dýchací cesty zajištěné tracheostomickou kanylou, což v tématu polykání představuje dva zásadní komplikující faktory. Závažnou komplikací tiché aspirace je aspirační pneumonie, která je spojena s vysokým rizikem morbidity a mortality. Teoretická část obsahuje teoretická východiska fyziologie polykání, patofyziologie poruch polykání se zaměřením na neurogenní dysfagii a neurologická onemocnění, která mohou mít vliv na polykání. Dále je zde zařazeno hodnocení polykání, vliv tracheostomie na polykání a dekanylace tracheostomické kanyly v kontextu dysfagie. Významnou součástí je i role sestry v managementu dysfagie a možnosti vzdělávání sester v této problematice. Praktická část je zpracována ve formě klinického doporučeného postupu, jehož výsledkem je příručka pro sestry. Příručka je zaměřena na problematiku neurogenní dysfagie, hodnocení polykání a management dysfagie.
