Úloha sestry intenzivistky v DAS algoritmu

Abstract

We are investigating this topic because there is no complex view of the role of an ICU nurse in difficult airway (DA). This is despite the fact that the nurse is an inseparable part of the team. In the case of inadequate management of the team securing the airway, DA can end up being fatal for the patient. It is necessary to realize that if the nurse knows her role, she can play an important part in the DA and minimize this risk. The main aim of our study was to find out the role of an ICU nurse in DA or in the DAS algorithm to minimize the permanent damage to the patient. The sample of our research consisted of 10 intentionally selected participants, working at the Clinic of Burn Medicine of the Královské Vinohrady University Hospital in the ICU and at the Clinic of Anaesthesiology and Resuscitation of the 3rd Faculty of Medicine of the Charles University and the Královské Vinohrady University Hospital. Our research is qualitative. Data collection was done by semi-structured interview method. Interviews with participants were recorded, then transcribed and presented using annotated transcription. We chose the grounded theory method as the method of analysis. Tématu se věnujeme (autorka práce spolu s vedoucí práce) proto, že doposud neexistuje komplexní pohled na roli sestry intenzivistky v obtížném zajištění dýchacích cest (OZDC). A to i přesto, že je sestra neoddělitelnou součástí týmu. OZDC může, v případě nedostatečného managementu výkon provádějícího týmu, skončit pro pacienta až fatálně. Je potřebné si uvědomit, že pokud by sestra svoji roli znala, může v OZDC sehrát důležitou roli a minimalizovat tak toto riziko. Hlavním cílem naší práce bylo zjistit, jakou roli sehrává sestra intenzivistka při OZDC, respektive v DAS algoritmu, aby minimalizovala trvalé poškození pacienta. Vzorek našeho výzkumu tvořilo 10 záměrně vybraných participantů, pracujících na Klinice popáleninové medicíny Fakultní nemocnice Královské vinohrady (KPM FNKV) na jednotce intenzivní péče (JIP) a na Klinice anesteziologie a resuscitace 3. LF UK a Fakultní nemocnice Královské Vinohrady (KAR FNKV). Náš výzkum je kvalitativní. Sběr dat proběhnul metodu polostrukturovaného rozhovoru. Rozhovory s participanty jsme nahrávaly, následně doslovně přepsaly a zobrazily pomocí komentované transkripce. Za metodu analýzy jsme si zvolily metodu zakotvené teorie.
