Diagnostika B-lymfoproliferativních onemocnění průtokovou cytometrií

Abstract

Souhrn Tato práce shrnuje postup diagnostiky vybraných B-lymfoproliferativních onemocnění v hematologické laboratoři metodou průtokové cytometrie. V teoretické části je popsána fyziologie a patofyziologie lymfopoézy, vysvětlen pojem lymfoproliferativní choroby, rozdělení B-Non Hodgkinových lymfomů, a jejich typické znaky používané k jejich diagnostice. Vybrané lymfoproliferace jsou popsány podrobněji. Praktická část je zaměřena na laboratorní diagnostiku lymfoproliferativních chorob. Sleduje se cesta vzorku jednotlivými hematologickými metodami, které napomáhají k diagnóze B-lymfomů. Mezi zmíněnými metodami je krevní obraz, diferenciál, nátěr kostní dřeně, cytochemie, a průtoková cytometrie. Následně je vysvětlen princip průtokové cytometrie a imunofenotypizace, přiblížen postup přípravy vzorku závislý na druhu materiálu, a popsána kontrola kvality metody. V závěru je vysvětleno hodnocení průtokové cytometrie, uvedené příklady vybraných lymfoproliferací a jejich vyhodnocení v programu Kaluza Analysis. This thesis summarizes the approach to the diagnostics of B lymphoproliferative diseases in the hematologic lab carried out by using the method of the flow cytometry. The theoretical part is focusing on the description of physiology and pathophysiology of lymphopoiesis, then on the explanation of the term lymphoproliferative illness, on the division of B NonHodgkin's lymphomas, and their typical traits used to diagnose them. The chosen lymphoproliferative disorders are described in greater detail. The practical part is focusing on the laboratory diagnostics of lymphoproliferative diseases. In this part, a sample is being observed as it is examined by specific hematologic methods, which are of help to the diagnosis of B lymphomas. The mentioned methods are blood count, morphological differential, bone marrow smear, cytochemistry and flow cytometry. Subsequently, the principle of flow cytometry and immunophenotyping is explained. That is followed by elaborating the process of preparation of the sample dependent on the type of material. Next there is described the control of the quality of the method. In the end there is explained the assessment of the flow cytometry, the given examples of chosen lymphoproliferative disorders, and their final evaluation in the program Kaluza Analysis. Keywords:...
