Coeliac disease, children, nutrition, gluten-free dietCeliakie je systémové autoimunitní onemocnění vyvolané lepkem vyskytující se u geneticky vnímavých jedinců. Klinické projevy celiakie se v posledních desetiletích změnily, jsou rozmanitější a začínají později. Ubylo dětí s klasickými gastrointestinálními projevy a přibylo dětí s neklasickými mimostřevními projevy. Vysoká senzitivita a specificita protilátek specifických pro celiakii umožnila u vybrané skupiny dětských pacientů s projevy onemocnění stanovit diagnózu celiakie bez biopsie. Jedinou léčbou celiakie je celoživotní přísná bezlepková dieta. Cílem práce bylo zjistit současný stav znalostí o celiakii u dětí, o její moderní diagnostice a především o léčbě. Informovat o problematice bezlepkové diety, o její legislativě, státní podpoře, příspěvcích zdravotních pojišťoven, sortimentu a dostupnosti bezlepkových potravin, jejich finanční náročnosti a o funkci pacientských organizací. V praktické části bylo dalším cílem zjistit okolnosti, za kterých byla celiakie u dětí diagnostikována a informovat o problémech spojených s udržováním bezlepkové diety v rodinách dítěte s celiakií. Metodikou práce bylo dotazníkové šetření. V anonymním dotazníku, který byl zaslán rodičům dětí s celiakií, bylo položeno celkem 33 otázek buď otevřenou, nebo uzavřenou formou. Coeliac disease is a systemic autoimmune disease caused by gluten occurring in genetically susceptible individuals. Clinical manifestations of coeliac disease have changed over past decades. It became more diverse and begins later. Children with common gastrointestinal symptoms are on decline while children with uncommon extraintestinal symptoms are on the rise. The high sensitivity and specificity of coeliac- specific antibodies in a group of pediatric patients with disease manifestation allowed diagnosis without biopsy. The currently used treatment for coeliac disease is lifelong strict gluten-free diet. The aim of the work was to determine the current state of knowledge of coeliac disease in children, available diagnostical methods, and treatment of coeliac disease. Furthermore, my goal was to inform about the issue of gluten-free diet, the legislation, state support, contributions of health insurers, the range and availability of gluten-free foods, their financial demands and the function of patient organizations.
