diabetes mellitus, vitamin D, glycated hemoglobinDiabetes mellitus 1. typu (DM1T) je chronické autoimunitní onemocnění vedoucí k progresivní ztrátě β-buněk pankreatu. Výskyt tohoto onemocnění ovlivňují genetické faktory a vlivy vnějšího prostředí. Jedním z možných negenetických faktorů asociovaných s rizikem DM1T je deficit vitaminu D. Cílem předložené bakalářské práce bylo stanovení sérových hladin glykovaného hemoglobinu-HbA1c a 25OH vitaminu D u dětských a dospělých pacientů Kliniky dětského a dorostového lékařství a 3. interní kliniky VFN a 1. LF UK s diagnózou diabetes mellitus 1. typu a jejich možných souvislostí. Soubor tvořilo 57 pacientů: 17 dětských pacientů s DM1T, 20 dospělých pacientů s DM1T a kontrolní skupina 20 dospělých pacientů s diabetes mellitus 2. typu DM2T. Pacienti přicházeli na pravidelné kontroly v rámci průběžné dispenzarizace. Sérové hladiny 25OH vitaminu D <10ng/ml byly hodnocené jako deficientní, 10-30 ng/ml jako insuficientní a 30-80 ng/ml jako dostatečné. Výsledky ukázaly, že se insuficience vitaminu D vyskytuje u všech skupin sledovaného souboru pacientů. Průměrná hodnota hladiny 25OH vitaminu D (25(OH)D) byla u dětských pacientů s DM1T 18,02±5,23 ng/ml, u dospělých pacientů s DM1T 20,95±10,83 ng/ml, dospělých pacientů s DM2T 17,88±11,16 ng/ml. Type 1 diabetes mellitus (DM1T) is a chronic autoimmune disease that leads to progressive loss of pancreatic β-cells. The incidence of this disease is effected by genetic factors and external environment. One of the possible non-genetic factors associated with the risk of DM1T is vitamin D deficiency. The purpose of this bachelor thesis was to determine the possible connections between serum levels of glycated hemoglobin-HbA1c and 25OH Vitamin D in pediatric and adult patients of the Clinic of Pediatric and Adolescent Medicine and 3rd Medical Department of the 1st Faculty of Medicine Charles University and General Fakulty hospital in Prague with diagnosis of diabetes mellitus 1.The group consisted of 57 patients: 17 pediatric patients with DM1T, 20 adult patients with DM1T and a control group of 20 adult patients with DM2T. Patients were randomly assigned to regular checkups as part of ongoing clinical examination. Serum levels of 25-OH vitamin D <10 ng/ml were evaluated as deficient, 10-30 ng/ml as insufficient and 30-80 ng/ml is sufficient. The results showed that vitamin D insufficiency occurs in all groups of the monitored patient population.
