Metformin, phenformin, energy metabolism in liver, oxidative stressJiž od středověku se k léčbě některých příznaků provázejících diabetes mellitus, používal extrakt z jestřabiny lékařské (Galega officinalis), která obsahuje derivát guanidinu galegin. Pozitivní účinky léčby však byly často převáženy množstvím negativních vedlejších efektů, které mohly vést až k úmrtí pacienta. Guanidin byl posléze nahrazen syntetickými deriváty biguanidy se sníženou toxicitou. Do klinických studií byly zařazeny pouze tři: metformin (N,N-dimetylbiguanid), fenformin (fenyletylbiguanid) a buformin (butylbiguanid), přičemž buformin a fenformin byly z klinického použití staženy kvůli závažnému riziku laktátové acidózy (Schäfer, 1983). Méně toxický metformin je v léčbě diabetu 2. typu široce využíván do současnosti a je označován jako lék prvé volby. V současnosti se pozornost opět obrací také k fenforminu, zejména v souvislosti s potenciálním využitím biguanidů jako kancerostatik i jako k vhodnému modelu pro experimentální studium účinku biguanidů. Navzdory dlouhodobému klinickému využití není mechanizmus působení biguanidů dosud plně objasněn. V současné době je obecně přijímána teorie, že hlavní příčinnou hypoglykemického účinku metforminu je inhibice glukoneogeneze v játrech, známy jsou však také jeho účinky na hnědou a bílou tukovou tkáň, svaly nebo protektivní vliv na srdce. The extract from the plant Galega officinalis containing the guanidine derivative galegin has been used in the treatment of diabetes-associated complications since middle ages. Nevertheless, the positive effects of the treatment were often overweight by the adverse side effects. Some sixty years ago guanidin was replaced by the less toxic synthetic biguanide derivatives - metformin, phenformin and buformin, the latter two being withdrawn due to the unacceptable risk of fatal lactate acidosis. Metformin is still widely used antidiabetics and belongs to the first choice drugs in the treatment of type 2 diabetes. Phenformin is now gaining renewed attention with regard to its antineoplastic properties. Despite its long-term clinical use the mechanism of biguanides action is not fully understood yet. At present it is generally accepted that the core of its antihyperglycemic effect lays in the inhibition of hepatic gluconeogenesis. In contrast, there is less consensus regarding the particular metabolic pathway or target that are responsible for the metformin-induced attenuation of gluconeogenesis.
