Pokud je poškození oka rozsáhlé a zasahuje do oblasti limbu, niky limbálních kmenových buněk (limbal stem cells, LSC), dochází k jejich deficienci a tím ke ztrátě schopnosti přirozené regenerace rohovky. Dochází k invazi spojivkového epitelu, k zánětu a neovaskularizacím a rohovka se postupně stává neprůhlednou. Samotná transplantace rohovky k léčbě nestačí. Navíc při poškození obou očí je nutná transplantace limbu nebo LSC od alogenního dárce a takový zákrok je nutný podpořit systémovým a lokálním podáním imunosupresivních léků, které mají rozsáhlé vedlejší účinky. Nalezením vhodné autologní buněčné náhrady by bylo možné dávky imunosupresiv vysadit a tím tyto vedlejší účinky odstranit. Mesenchymální kmenové buňky (mesenchymal stem cells, MSC) představují vhodnou alternativu z důvodu svých imunomodulačních a diferenciačních vlastnostní. Z tukové tkáně nebo kostní dřeně lze získat dostatečné množství MSC. Naším cílem bylo sledovat lokální produkci cytokinů a dalších molekul spojených se zánětem po chemickém poškození myší rohovky a po léčbě kmenovými buňkami transplantovanými pomocí nanovlákenného nosiče na poškozený povrch oka. The damage of ocular surface represents one of the most common causes of decreased quality of vision or even blindness. If the injury is extensive and includes the region of limbus, niche of limbal stem cells (LSC), LSC deficiency occurs and the natural corneal regeneration is stopped. Conjunctival epithelium migrates into the injured area. Neovascularization, local inflammation and corneal opacity occur. Corneal transplantation is an insufficient treatment in such case. If the injury is bilateral, the allogenic limbal graft or LSC transplantation is required. In such cases systemic immunosuppressive drugs with many negative side-effects must be administered. The search for an adequate autologous substitution is important for avoid immunosuppressive medication. Mesenchymal stem cells (MSC) represent a perspective substitution for the reason of their immunomodulatory properties and the capability to differentiate in many cell types. There is possibility to isolate sufficient number of these cells from adipose tissue or bone marrow which are relatively easily accessible.
