Disertační práce Přínos repetetivní transkraniální magnetické stimulace v terapii specifických příznaků schizofrenie Tomáš Novák Psychiatrické centrum Praha 2011 SOUHRN: Transkraniální magnetická stimulace (TMS) je moderní, neinvazivní metoda založená na principu elektromagnetické indukce, která nachází uplatnění ve výzkumu, diagnostice a léčbě řady neuropsychiatrických poruch. Dosavadní terapeuticky orientované studie naznačují, že repetetivní TMS (rTMS) by mohla najít uplatnění také v léčbě některých příznaků schizofrenie. Úvodní část předkládané práce je věnována popisu TMS, předpokládanému mechanismu účinku a zásadám použití TMS v klinické praxi. Dále obsahuje základní informace o schizofrenii a o výsledcích dosavadních klinicky orientovaných studií s rTMS u pacientů se schizofrenií. V další části práce jsou prezentovány výsledky tří studií, jejichž cílem bylo ověřit terapeutickou účinnost rTMS u specifických příznaků schizofrenie, konkrétně u negativních příznaků a sluchových halucinací. Ve studii 1, randomizované, dvojitě slepé, paralelní, placebo stimulací kontrolované studii, jsme nepotvrdili původní předpoklad, že vysokofrekvenční rTMS aplikovaná nad levým dorzolaterálním prefrontálním kortexem povede k zmírnění negativních příznaků či zlepšení kognitivních funkcí u pacientů se schizofrenií. The TMS is widely used in research, diagnostics and treatment of neuropsychiatric disorders. Previous studies imply that repetetive TMS (rTMS) might be useful also in the treatment of some specific symptoms of schizophrenia. The thesis consists of three studies focused on the evaluation of the therapeutic effect of rTMS on specific symptoms of schizophrenia, particularly on negative symptoms and auditory hallucinations. The double-blind sham-controlled study of high-frequency rTMS (20Hz) for negative symptoms in schizophrenia The high-frequency repetitive transcranial magnetic stimulation (HF-rTMS) over the prefrontal cortex is a promising method for the treatment of negative symptoms of schizophrenia.
