Urethra, urethral perfusion, urinary incontinence, ultrasound, polypropylen implants, shrinkageÚvod: Kontinence moči u žen je zajišťována zevním a vnitřním mechanismem. Vnitřní mechanismus, tj. vliv vlastní stavby uretry, inervace, perfuze submukozních plexů apod., nejsou v klinické praxi vyšetřovány. Dlouhodobě je známa souvislost inkontinence a rekonstrukčních výkonů. Nedílnou součástí všech reparačních procesů jsou faktory ischemie a neovaskularizace. Můžeme očekávat, že hojení a jizvení může mít vliv na vnitřní uzávěrový mechanismus. Rekonstrukční výkony se syntetickými implantáty patří mezi operace, kde se jizvící proces projevuje ve větší míře. Metodika: Do studie jsme zařadili pacientky s defektem předního kompartmentu a randomizovali je do 3 skupin dle operační techniky a inzerce implantátu. Pacientky byly vyšetřeny před a po operaci ultrazvukem, včetně Dopplerovského vyšetření perfuze, a urodynamického vyšetření. Pooperační ultrazvukové vyšetření bylo provedeno 4. pooperační den a 3-5 měsíců po operaci. Byla studována korelace ultrazvukových a urodynamických parametrů. Výsledky: Celkem jsme zařadili 87 pacientek. Neprokázali jsme korelaci mezi morfologickými změnami způsobenými operacemi a stupněm inkontinence. Metodika hodnocení perfuze oblasti uretry a uretrovezikální junkce pro svou nepřesnost znemožnila korelaci s ostatními parametry. Maximální uretrální tlak nekoreloval s...
Introduction: Intrinsic and extrinsic urethral factors play a significant role in urinary continence mechanism in women. Urethral wall structure including inervation, perfusion of submucosal layer etc. is not clinically assessed despite its important role in urethral closure function. The association of incontinence and pelvic floor reconstructive surgery is well known. Every postoperative healing process is associated with factors of ischemia and neovascularisation. According those facts we would expect that the healing and scaring should involve intrinsic urethral mechanism. After reconstructive surgery Implants further increase scaring process. Methods: In our study we included patients with anterior compartment defect. We randomized patients into three interventional arms according the surgical approach and use of implants. Before and 3-5 month after the surgery we performed urodynamic studies and pelvic floor ultrasound examination, including Doppler for urethral perfusion assessment. Another early ultrasound scan was added forth day after surgery. We correlated ultrasound and urodynamic parameters. Results: We randomized 87 patients.
