Souhrn Terapie Crohnovy nemoci (CN) u dětí se neliší významněji od léčby dospělých - i zde hrají dominantní úlohu systémové kortikosteroidy (SKS) společně s imunosupresivní léčbou. Cíle: 1) posouzení efektivity a hepatotoxicity 6-thioguaninu (6-TG) na animálním modelu experimentální kolitidy 2) stanovit čas do nástupu ustálených hladin 6-thioguaninových nukleotidů po zahájení léčby azathioprinem 3) zhodnocení aplikované dávky SKS včetně kalkulace kumulativní dávky v 8., 12. a 52.týdnu léčby ve vztahu k aktivitě CN 4) analýza SKS šetřícího efektu azathioprinu v 52. týdnu léčby. Soubor a metoda: Ad 1) U 50 laboratorních myší (BALb/c F) byla indukována experimentální kolitida a zahájena léčba 6-TG, azathioprinem a placebem. Po 14 dnech byl pokus ukončen. Ad 2) vstupní kritéria byla dg. nespecifického střevního zánětu (NSZ) a zahájení léčby azathioprinem, stanovení hladin bylo provedeno metodou HPLC. Ad 3-4) Předmětem studie byli pacienti, kteří splňovali vstupní kritéria: věk 0-19 let, diagnóza CN, prospektivní hodnocení aktivity nemoci, sledování umožňovalo získat veškeré potřebné informace, které vyžadovala metodika studie. Crohn disease (CD) treatment in children doesn't differ from adults today. The main role in management of disease play systemic corticosteroids (CS) and immunesupressants. Aims: 1) evaluate efficacy and hepatotoxicity of 6-thioguanine (6-TG) on mice model of experimental colitis. 2) evaluate time to steady state of 6-thioguanine nucleotides after azathioprine (AZA) treatment initiation in children with inflammatory bowel disease (IBD). 3) evaluate dose (incl. cumulative dose) of CS applied through the first year after diagnosis according to disease activity. 4) AZA steroid sparing effect analysis in week 52 of follow up. Methods: 1) Dextran sodium sulfate colitis was induced in 50 mice (BALb/c F) for 9 days, treatment with placebo, AZA and 6-TG started at once. Study was terminated on day 14. 2) inclusion criteria: new IBD diagnosis, AZA treatment. Steady state analysis was performed by high performance liquid chromatography. 3-4) inclusion criteria: age 0-19, CD (according to Porto criteria), prospective disease activity assessment, follow-up at least 52 weeks.
