Disociace a komplexní posttraumatická stresová porucha

Abstract

Tato bakalářská práce se zabývá tématem vztahu mezi disociacemi a komplexní posttraumatickou stresovou poruchou (CPTSD). CPTSD představuje novou diagnostickou jednotku, zahrnutou v 11. revizi MKN, která je v České republice platná od února 2024. První kapitola práce se zabývá vymezením pojmu trauma, a dále přibližuje fenomén komplexního traumatu a jeho dopady v raném dětství. Druhá kapitola představuje nová diagnostická kritéria PTSD a CPTSD, a dále popisuje odlišnosti v projevech obou poruch, prevalenci, diferenciální diagnostiku a doporučení pro léčbu CPTSD. Třetí kapitola se věnuje disociaci jako komplexnímu transdiagnostickému fenoménu. Představeny jsou především patologické formy disociace a diskutovány jsou také různé modely, které vysvětlují jejich vznik. Ve čtvrté kapitole jsou propojena témata komplexního traumatu, disociací a CPTSD. Diskutovány jsou zejména aktuální výzkumy, které se zaměřují na vztah mezi disociativními projevy a CPTSD. Empirickou část práce tvoří návrh výzkumného projektu, který si klade za cíl ověřit, zda jedinci splňující diagnostická kritéria CPTSD vykazují signifikantně vyšší míru psychoformní i somatoformní disociace oproti jedincům, kteří kritéria této poruchy nesplňují. This bachelor thesis deals with the topic of the relationship between dissociation and complex posttraumatic stress disorder (CPTSD). CPTSD represents a new diagnostic unit, included in the 11th revision of the ICD, which is valid in the Czech Republic since February 2024. The first chapter of the thesis deals with the definition of the term trauma, and further introduces the phenomenon of complex trauma and its impact in early childhood. The second chapter introduces the new diagnostic criteria for PTSD and CPTSD, and describes the differences in manifestations of both disorders, prevalence, differential diagnosis and recommendations for treatment of CPTSD. The third chapter focuses on dissociation as a complex transdiagnostic phenomenon. In particular, pathological forms of dissociation are presented and various models that explain its origins are also discussed. In the fourth chapter, the topics of complex trauma, dissociation and CPTSD are linked. Especially current research that focuses on the relationship between dissociative experience and CPTSD is discussed.
