Úvod V dnešní době se sedace stala nezbytnou součástí klinické praxe. Navzdory této skutečnosti nám stále chybí data popisující přesný dopad sedace na srdeční funkci. Cíl práce Studie v disertační práci si klade za cíl porovnat změny srdeční funkce, indukované po sedaci midazolamem nebo dexmedetomidinem, pomocí magnetické rezonance (MRI). Celkový počet 30 dobrovolníků byl randomizován do dvou skupin: 15 pacientů ve skupině midazolamu (MID) a 15 pacientů ve skupině dexmedetomidinu (DEX). Každý účastník podstoupil během jednoho sezení vyšetření srdce pomoci MRI, a to před a po podání sedace. Byly zaznamenány následující parametry: tepový objem pravé (Pul-vol) a levé komory (Ao- vol), maximální rychlost krevního proudu přes aortální (Ao-flow) a pulmonální chlopeň (Pul- flow) během systoly a maximální rychlost krevního průtoku přes mitrální chlopeň během časné (E-diast) a pozdní fáze diastoly (L-diast). Monitor zaznamenával hodnoty středního krevního tlaku (MAP), pulsu (P) a saturace krve kyslíkem (SpO2) v 5minutových intervalech. Výsledky Parametry systolické funkce komor byly u dobrovolníků s dexmedetomidinem významně sníženy: Ao-vol (p = 0,006), Pul-vol (p = 0,003), Ao-flow (p = 0,048), Pul-flow (p = 0,007). Midazolam stejně významně snížil Ao-vol (p = 0,001), Pul-vol (p = 0,01), a Ao-flow (p = 0,04)....
Introduction Nowadays, sedation has become an essential part of clinical practice. Despite this fact, we still lack data describing the exact impact of sedation on cardiac function. Objective The aim of this work is to compare the changes in cardiac function induced after sedation with midazolam or dexmedetomidine by magnetic resonance imaging (MRI). A total of 30 volunteers were randomized into two groups: 15 patients in the midazolam (MID) group and 15 patients in the dexmedetomidine (DEX) group. Each participant underwent a one session MRI imaging of the heart during and after sedation administration. The following parameters were recorded: right ventricular (Pul-vol) and left ventricular (Ao-vol) stroke volume, maximal blood flow velocity through the aortic (Ao-flow) and through the pulmonary valve (Pul-flow) during systole, and maximal blood flow velocity through the mitral valve during early (E-diast) and late diastole (L-diast). The monitor recorded mean blood pressure (MAP), pulse (P), and blood oxygen saturation (SpO2) at 5minute intervals.
