Medical intensive care team, palliative care, termoinal extubation, palliative care communication, transition from full therapy to palliative care(v ČJ) Úvod: I přesto, že by se na první pohled mohlo zdát, že paliativní péče není součástí intenzivního prostředí, opak je pravdou. Paliativní přístup k nemocným, kteří se nacházejí na resuscitačních odděleních či jednotkách intenzivní péče, patří do celku komplexního přístupu v ošetřování těžce nemocných. Zahrnuje individuální a citlivý přístup k pacientovi, který se nachází ve stavu, kdy jsou vyčerpány všechny dostupné možnosti léčby, a další proaktivní kroky v léčení by znamenaly utrpení pacienta, strádání jeho rodiny a v neposlední řadě by přístup léčby bez paliativního konceptu měl i negativní dopad na celý ošetřující personál. Paliativní péče tedy zahrnuje nejen individuální přístup k potřebám pacienta, ale také jeho rodiny a blízkých. Nejedná se o koncept péče založený na přístupu jednotlivce k nemocnému, ale naopak na spolupráci celého ošetřujícího týmu, tedy lékařů, zdravotních sester a zdravotnických záchranářů. Paliativní tým působící v nemocnici, je k dispozici pracovníkům nejen intenzivního oddělení, ale i celé nemocnice, a tvoří ho interdisciplinární odborníci. Funkční paliativní koncept na intenzivním oddělení, znamená přínos pro pacienta a jeho rodinu. Ačkoliv je péče úzce spjatá s negativní součástí životní cesty pacienta. (Aj) Introduction: Although palliative care may not seem to be part of an intense environment, it is true. Palliative approach to patients who are in resuscitation wards or intensive care units is part of a comprehensive approach in the treatment of seriously ill patients. It includes an individual and sensitive approach to a patient in a state where all available treatment options are exhausted, and further proactive steps in treatment would entail patient suffering, family hardship and, last but not least, a non-palliative treatment approach would have a negative impact to the entire nursing staff. Palliative care therefore involves not only an individual approach to the needs of the patient but also of the patient's family and loved ones. It is not a concept of care based on an individual's approach to the patient, but rather on the cooperation of the entire nursing team, ie doctors, nurses and paramedics. The palliative team working in the hospital is available to staff not only in the intensive department but also in the entire hospital, and is made up of interdisciplinary experts.
