Neoadjuvantní terapie u paientů s karcinomem jícnu a její vliv na funkci ledvin

Abstract

Název diplomové práce: Neoadjuvantní terapie u pacientů s karcinomem jícnu a vliv terapie na funkci ledvin Cílem této práce je porovnání používaných vzorců pro odhad glomerulární filtrace. Vybranou skupinu pacientů reprezentují pacienti s časným stádiem karcinomu jícnu, kteří jsou léčeni neoadjuvantní terapií. Tuto terapii tvoří radioterapie a chemoterapie, kterou tvoří cisplatina a fluorouracil. Cisplatina má nefrotické účinky. Metodika se snaží co nejpřesněji popsat aktuální stav pacienta. Charakteristika pacienta se skládá z biochemického rozboru, anamnézy pacienta a rozboru renálních funkcí. Budeme sledovat vliv komorbidit a konkomitantní farmakoterapii na funkci ledvin. Výsledky tvoří vzájemné porovnání jednotlivých výpočtů. Každý pacient má vytvořenou vlastní renální anamnézu, včetně porovnání jednotlivých odhadů glomerulární filtrace. Celkový počet pacientů je ovlivněn nepříznivou prognózou, účastí některých z nich v klinických studiích a rychlou progresí onemocnění. Výstupem z této práce bude porovnání odhadů glomerulární filtrace a její praktické uplatnění při výpočtu podávaných cytostatik.

The selected group of patients are patients with early oesophageal cancer who are treated with neoadjuvant therapy. This therapy consists of radiotherapy and chemotherapy, which consists of cisplatin and fluorouracil. Cisplatin has nephrotic effects. Methodology strives to describe as accurately as possible the current condition of the patient. The patient's characteristics consists of a biochemical analysis, a anamnesis of the patients and an analysis of the renal function. The effect of comorbidities and concomitant pharmacotherapy on renal fiction is monitored. Results of this work provide a comparison of individual calculations. An individual renal anamnesis has been created for each patient, including a comparison of individual glomerular filtration estimates.
