Mezi nejvýznamnější komplikace vyvolané vedlejšími účinky CNI patří arteriální hypertenze a porucha funkce ledvin. Cílem práce bylo ověřit, zda kombinace antihypertenziv zahrnující duální blokádu renin-angiotenzinového systému (RAS) bude stejně účinná v léčbě hypertenze jako standardně podávaná kombinace antihypertenziv. Druhou hypotézou bylo posoudit, zda účinná kombinační léčba hypertenze (duální RAS nebo standardní kombinace antihypertenziv) povede ke zmírnění progrese chronického onemocnění ledvin u pacientů s dlouhodobou imunosupresivní profylaxí. Léčba arteriální hypertenze zahrnující kombinaci inhibitoru angiotenzin konvertujícího enzymu (ACEi) a antagonisty receptorů pro angiotenzin II (AT1 blokátoru) byla stejně účinná ve srovnání se standardně podávanou kombinací antihypertenziv. V obou studiích bylo dosaženo cílových hodnot krevního tlaku. Podávání kombinace antihypertenziv zahrnující duální blokádu RAS vedlo ke zmírnění progrese chronického onemocnění ledvin jak v experimentu, tak v klinické části, kde se nefroprotektivní efekt zesílené blokády RAS projevil příznivějšími výsledky glomerulární filtrace a výrazným snížením albuminurie. The introduction of calcineurin inhibitors (CNI) into immunosuppressive regimens significantly improved patients prognosis after heart transplantation. Some of the most significant complications have been recognized, such as the development of arterial hypertension and renal impairment due to calcineurin inhibitor toxicity. The aim of the study was to compare the effect of the dual blockade of the renin-angiotensin system (dual RAS combination) with standard antihypertensive medication on blood pressure control. The second aim was to evaluate whether effective antihypertensive combination therapy (dual RAS or a standard antihypertensive drugs combination) would reduce the progression of chronic kidney disease in patients with chronic immunosuppressive prophylaxis. Treatment of arterial hypertension involving the combination of angiotensin-converting enzyme inhibitor (ACEi) and angiotensin II receptor blocker (ARB) was similarly effective compared to the standard combination of antihypertensives.
