Modely paliativní péče.

Abstract

Hlavním tématem mé práce je umírání a to jak v historii, tak v současnosti s ohledem na možnost domácího ošetřování za použití paliativní péče. Možnost pečování o nemocného v rodinném kruhu by mělo být v dnešní době samozřejmostí stejně tak, jako tomu bylo kdysi. V diplomové práci nabízím pohled na umírání a zásvětí ve starověkém Egyptě a Řecku, kde popisuji jak se nemocný připravoval na svůj skon či jaké existovaly pohřební rituály. Nedílnou součástí je také pojetí zásvětí u těchto dvou kultur, protože představa toho, co bude po smrti velmi ovlivňovala tehdejší život, nemoc i smrt. V druhé části práce rozebírám stručně historii péče o nemocné a zrod institucionální péče, počátky hospicového hnutí a jeho myšlenku na kterou navazuje rozbor paliativní péče, jako návrat k důstojné péči o umírající. Zabývám se i tématem lékařské etiky, eutanázie a dystanázie. Vzhledem k tomu, že domácí péče není vždy pro pacienta možná, se v posledních kapitolách zabývám rozborem péče ve zdravotnických zařízeních, kde čerpám mj. z vlastní zkušenosti a hledám možná zlepšení tak, aby i v těchto podmínkách bylo o těžce nemocné a umírající důstojně a s citem postaráno.

The main thema of my work is dying both in the history and at present in relation to the possibility of a home care by means of the palliative care. The possibility to take care of the patient in the family circle should be an obviosity today the same way as it was in the past. In this thesis I offer a view of dying and the other world in ancient Egypt and Greece, describing how an ill man was being prepared for his death or what funeral rituals existed. An integral part of this work is also the conception of the other world of these two civilizations because the vision of that coming after the death influenced very much those days life, illness and death. In the second part of my work I analyze in brief the history of the care for the ill people and the birth of institutional care, the beginnings of the hospice movement and its ideals followed by the analysis of paliative care as the comeback to the dignified care of deathward. I am also concerned with the thema of medical ethics, euthanasia and dystanasia. As the home care of the patient is not possible every time, in the last chapters I analyze the care in medical facilities, where I also draw my own experience and look for possible improvements so that even in these conditions the seriously ill and deathwards should be treated with a dignity nad heart.
