dc.description.abstract | Úvod: Nedílnou součástí anesteziologické péče v průběhu chirurgických výkonů je aplikace inhalačních a intravenózních anestetik. Obě skupiny mají výrazný vliv na kardiovaskulární systém, a to jak ovlivněním dodávky a spotřeby O2 v myokardu, tak ovlivněním kardiomyocytů na úrovni buněčných membrán, iontových kanálů a regulačních enzymů. Cíl: 1. Zjistit, jak různé koncentrace izofluranu ovlivňují rozměry levé komory (LV) a její systolickou funkci u laboratorního potkana. 2. Zhodnotit vliv izofluranem navozeného preconditioningu myokardu (APC) na odolnost srdečního svalu k ischemicko-reperfuznímu (I-R) poškození. 3. Porovnat vliv anestezie, založené na kombinaci ketamin - dexmedetomidin (KET-DEX), na koncentraci kardiospecifických biomarkerů a časný klinický pooperační průběh s anestezií, založenou na kombinaci sevofluran - sufentanil (SEVO), u pacientů podstupujících elektivní chirurgickou revaskularizaci myokardu (CABG). Metodika: 1. Provedli jsme transtorakální echokardiografické vyšetření při imobilizaci potkanů izofluranem v koncentraci 1,5 - 3 %. 2. Po navození APC izofluranem (0,5 a 1 MAC) jsme sledovali komorové arytmie na modelu izolovaného perfundovaného srdce potkana, a to během regionální ischemie (45 min.), vyvolané okluzí ramus interventricularis anterior, i následné reperfuze (60 min.);... | cs_CZ |
dc.description.abstract | Background: Surgical procedures are invariably accompanied by the use of inhalational and intravenous anesthetics. Both groups have strong influence on cardiovascular system by the interaction with myocardial oxygen supply/demand ratio and cardiomyocyte functions at the level of cell membranes, ion channels and regulatory enzymes. Aims: 1. To examine the effects of different isoflurane concentrations on the left ventricular (LV) dimensions and systolic function in the rat. 2. To examine the effects of isoflurane-induced myocardial preconditioning (APC) on the cardiac tolerance to ischemia- reperfusion (I-R) injury. 3. To compare the influence of anesthesia, based on ketamine- dexmedetomidine (KET-DEX), on the release of biochemical markers of myocardial injury and the early postoperative course with the anesthesia, based on sevoflurane-sufentanil (SEVO), in the patients undergoing coronary artery bypass grafting (CABG). Methods: 1. We carried out transthoracic echocardiographic examination in the rats immobilized by 1.5-3% concentration of isoflurane. 2.
