Poruchy hybného systému pacientů v dlouhodobé remisi nádorových onemocnění vzniklých v dětském věku /NEFROBLASTOM/

Abstract

Musculoskeletal System Dysfunction in Patients in Long-Term Remission of Wilms Nephroblastoma Objective: The objective of this retrospective clinical study was to describe the multidisciplinary program of childhood cancer survivors in their young adult age in an adult-based ambulatory medical setting and to report the latent sequelae of the pathology. The hypothesis is that the treatment with all three modalities (chemotherapy, radiotherapy and surgery) causes the development of the latent effects in varying degrees. The goal of this study was to investigate the typical musculoskeletal system changes of survivors of Wilms Nephroblastoma who had previously received their basic treatment more than 5 years ago. Design: 151 subjects underwent clinical assessment of body posture, postural tests and evaluation on BalanceMaster® force plate. These tests were conducted in conjunction with other clinical evaluations, namely, pulmonary and renal function tests, immunology deficiency and exercise capacity to seek its relationship to the diseases' risk factors. The correlation between postural findings and the above clinical tests were investigated. SOUHRN Cílem práce bylo popsat a strukturovat pozdní následky na pohybovém aparátu u pacientů, kteří byli v dětství léčeni pro nefroblastom (NFB). Pacienti podstoupili terapii, skloženou ze tří typů léčby: chemoterapie, radioterapie a chirurgická léčba. Dílčí cíle: Na relativně homogenní skupině pacientů (s jediným typem základního onemocnění léčených v akutní fázi) popsat typické posturální poruchy hybného systému rozvinuté během pěti a více let odstupu od ukončení kombinované léčby (chirurgické, chemo- a radioterapie). V návaznosti na tato zjištění navrhnout pro pacienty bezprostředně po léčbě i po delší časové prodlevě systém prevence poruch hybného systému vznikajících jako sekundární následky léčby primární diagnózy. Porovnat funkční poruchy hybného systému a funkční poruchy plicních funkcí u pacientů s/bez radioterapie. Posoudit dopad poruch hybného systému na tělesnou zdatnost jedince. Práce je klinickou průřezovou studií s retrospektivním dohledáním dat pacientů léčených pro NFB v letech v době od 1.1.1980 do 31.12.2004 na Klinice dětské onkologie (dnes Klinice dětské hematologie a onkologie UK-2.LF a FN Motol).
