The Influence of Selected Nutritional and Risk Factors on Postoperative Complications in Oncological Patients After Microsurgical Reconstruction in the Orofacial Region

dc.description.abstractKarcinomy orofaciální oblasti mají stoupající incidenci ve společnosti, především v mladších věkových kategoriích. Jednou ze zásadních terapeutických metod odstranění novotvaru je mikrochirurgická rekonstrukce. Jedná se o velmi složitý a dlouhý zákrok, který však může prodloužit pacientův život a významně zlepšit jeho kvalitu. Rekonvalescence po mikrochirurgickém zákroku v orofaciální oblasti bývá často zdlouhavá a náročná, zejména kvůli rozsahu operačního výkonu, nutnosti složité rekonstrukce a častému výskytu komplikací. V tomto období je klíčová jak pečlivá pooperační péče, tak včasné rozpoznání nutričních a zánětlivých parametrů, které mohou ovlivnit celkový vývoj pacienta. Pro tuto retrospektivní studii bylo vybráno 120 pacientů, kteří podstoupili v letech 2018- 2025 mikrochirurgickou rekonstrukci na Stomatologické klinice VFN. Údaje o těchto pacientech byly čerpány z lékařských chorobopisů a ze systému MEDEA. Sledovanými parametry byly pooperační hladiny albuminu a CRP, BMI, anamnéza kouření a užívání alkoholu. Byla zjištěna významná negativní korelace mezi hladinou albuminu a CRP (ρ = −0,68; p <0,001). Pacienti s komplikacemi vykazovali kritickou kombinaci hypoalbuminémie (<26 g/l) a vysoké zánětlivé aktivity (CRP >170 mg/l). Podváha (BMI <18,5) a anamnéza abúzu byly identifikovány jako...cs_CZ
dc.description.abstractCarcinomas of the orofacial region show a rising incidence in society, particularly among younger age groups. Microsurgical reconstruction is a fundamental therapeutic method for tumor resection. Although it is a highly complex and lengthy procedure, it can extend the patient's life and significantly improve its quality. Recovery after microsurgical intervention in the orofacial region is often lengthy and demanding, primarily due to the extent of the surgical procedure, the complexity of the reconstruction, and the frequent occurrence of complications. During this period, meticulous postoperative care and the early identification of nutritional and inflammatory parameters that may influence the patient's overall progress are crucial. For this reason, this study focuses on monitoring serum albumin levels and their correlation with the occurrence of postoperative complications. For this retrospective study, 120 patients were selected who underwent microsurgical reconstruction at the Department of Stomatology, General University Hospital (VFN) between 2018 and 2025.
