Riziko rozvoje diabetes mellitus 2. typu u žen s gestačním diabetes mellitus

Abstract

This bachelor's thesis deals with the increasingly progressing issue of the development of type 2 diabetes mellitus in connection with gestational diabetes mellitus. The theoretical part focuses on the etiopathogenesis, diagnosis and treatment of type 2 diabetes mellitus and gestational diabetes mellitus. It also adresses the acute and long-term complications of gestational diabetes for both the moter and her offspring. In the practical part, a cohort of 400 patients from the Clinic of Gynecology, Obstetrics and Neonatology of the First Faculty of Medicine, Charles University and General Hospital in Prague was established. These patients were monitored for a history of gestational diabetes mellitus in the diabetology outpatient clinic for pregnant women. Using statistical data analysis, data leading to the objectives and hypotheses of the thesis were determined. The objectives included identifying patient compliance with postpartum oral glucose tolerance testing (oGTT), the influence of pharmacological treatment on the future development of type 2 diabetes mellitus, and the association between age, weight, family history, and the development of gestational diabetes mellitus as a significant risk factor for type 2 diabetes mellitus. Tato bakalářská práce se věnuje stále progredující problematice rozvoje diabetes mellitus 2. typu ve spojitosti s gestačním diabetes mellitus. Teoretická část je zaměřena etiopatogenezi, diagnostiku a léčbu diabetes mellitus 2. typu a gestační diabetes mellitus. Zaměřuje se i na akutní a dlouhodobé komplikace gestačního diabetu pro matku i jejího potomka. V praktické části byl stanoven soubor 400 pacientek z Kliniky gynekologie, porodnictví a neonatologie 1LF UK a VFN v Praze, které byly sledovány pro anamnézu GDM v diabetologické ambulanci pro těhotné ~eny. Metodou statistické analýzy dat byly stanoveny údaje vedoucí k cílům a hypotézám práce. Cíle obnáaely zjiatění compliance pacientek ke kontrolnímu oGTT po porodu, vlivu farmakologické léčby na budoucí rozvoj DM2 a spojitost mezi věkem, hmotností, rodinnou anamnézou a rozvojem samotného GDM jako~to výrazného rizika pro rozvoj DM2. K tomu byly pou~ity údaje z nemocničního informačního systému Medea. Účast ~en na kontrolním oGTT po porodu byla velmi nízká, dostavilo se pouze 18 % ~en z celkového vzorku. }eny léčené farmakologicky měly mnohonásobně zvýaené riziko poporodního rozvoje prediabetu nebo diabetu, oproti ~enám léčených pouze úpravou stravy a pohybu.
