Změny střevního mikrobiomu u pacientů s idiopatickými střevními záněty

dc.description.abstract(EN) Microbes have coevolved with humans forming symbiotic communities that constantly challenge the immune system and, when imbalanced, could lead to diseases like inflammatory bowel disease (IBD). Patients with IBD suffer from microbial dysbiosis and chronic inflammation which could be potentiated by immune system reaction to the commensal microbiota. In the research presented here, firstly I have focused on the description of the gut and skin microbiome from patients with IBD and secondly, I investigated the process of antimicrobial defense. Patients with IBD on two different biological therapies targeting TNFα, IL-12 and IL-23 cytokines were tracked for changes in their gut and skin microbiome features. Although, neither differences in gut microbial diversity nor composition were linked with the progression of the therapies, an increased similarity to the healthy control group at week 38 of anti-TNFα therapy was found. This shift in microbiome could be considered beneficial and could be attributed to the inflammation reducing effect of the therapy. While analyzing the microbiome features, various patient characteristics were taken into account and the sources microbiome of variability were uncovered, out of which the interindividual variability stood out the most. dc.description.abstract(CZ) Mikrobi se vyvíjeli společně s jejich lidskými hostiteli a vytvořili s nimi symbiotický celek, což ale představuje náročný úkol pro imunitní systém. Pacienti s ISZ trpí mikrobiální dysbiózou a chronickým zánětem, který může být způsoben reakcí imunitního systému na komenzální mikrobiotu. Ve zde prezentovaném výzkumu jsem se nejprve zaměřila na popis střevního a kožního mikrobiomu pacientů s ISZ, a na proces antimikrobiální obrany. Pacienti s IZS na dvou různých biologických terapiích cílící na cytokiny TNFα, IL-12 a IL-23 byli sledováni na výskyt změn v jejich střevním a kožním mikrobiomu. Ačkoli s průběhem sledovaných biologických terapií nebyly spojeny změny v diverzitě ani složení střevního mikrobiomu, v týdnu 38 anti-TNFα léčby byla zjištěna zvýšená podobnost se zdravou kontrolní skupinou. Tento posun ve složení mikrobiomu by mohl být považován za prospěšný a mohl by být připsán účinku terapie, tedy snížení zánětu. Při analýze mikrobiomu byly zohledněny různé charakteristiky pacientů a byly odhaleny zdroje variability mikrobiomu, z nichž nejvíce vynikla interindividuální variabilita. Pokud jde o kožní mikrobiom na místech bohatých na mazové žlázy, byly zjištěny rozdíly ve složení a diverzitě mezi zdravými jedinci a pacienty s ISZ.
