Systém financování zdravotní péče v České republice

Abstract

The system of financing health care in the Czech Republic This thesis aims to describe and assess the relationship between health insurance companies and healthcare providers. It focuses on its imbalances, dominance by health insurance companies, as well as the impact of this dominance on the patient and the entire Czech Republic health care system. This issue has become very acute at present because of the accentuation of health policy priorities by political leaders, especially it is often a very important topic of local politics, and also because of the penetration of business into healthcare. The next aim of this study is to conclude the long-term sustainability and balance approach of the current reimbursement system for providers of healthcare services, as well as outlining other possible scenarios. The way to this conclusion is a focus on the description of the current sources of health care provision, focus on its financing and the analysis of the legal regulation of the whole issues, as well as a summary of the determining court findings. The outcome is in the draft of legislative changes that should remedy the distortion in one of the segments of the providing of health services, namely in providing Follow-up intensive, Follow-up ventilation and Long-term intensive nursing care. Tato rigorózní práce si klade za cíl popsat a posoudit vztah mezi zdravotními pojišťovnami a poskytovateli zdravotních služeb se zaměřením na jeho nevyrovnanost, dominanci ze stran zdravotních pojišťoven, jakož i dopad této dominance na pacienta a celý systém zdravotnictví v České republice. Tato problematika se stala v současnosti velmi ožehavou z důvodu akcentování priority zdravotnictví ze strany politických představitelů, kdy toto je často velmi významným tématem lokální politiky, a též z důvodu pronikání businessu do zdravotnictví. Cílem této práce je následně dojít k závěru o dlouhodobé udržitelnosti a vyrovnanosti přístupu současného systému úhrad k poskytovatelům zdravotních služeb, jakož i nastínění jiných možných scénářů vývoje. Cestou k tomuto závěru je pak zaměření se na popis současných východisek poskytování zdravotní péče, zaměření se na její financování a analýzu právní úpravy celé problematiky, jakož i shrnutí určujících soudních nálezů. Vyústění pak nachází v návrhu legislativních změn, jež by měly napravit distorzi v jednom ze segmentů poskytování zdravotních služeb, a to v poskytování následné intenzivní, následné ventilační a dlouhodobé intenzivní ošetřovatelské péče.
